Ultrasound Diagnostic Clinical Value Of Leak Complications After Abdominal Aortic Aneurysm Surgery

Aneurysm is the most common vascular disease causing disability or death.artery aneurysm occurs in the entire body,the most common is located in the abdominal aorta aneurysm.The onset of abdominal aortic aneurysm is more covert,commonly found by physical examination.Abdominal aortic aneurysm is the main threat to rupture,fracture rate of 2.2% a year.If abdominal aortic aneurysm ruptured,the mortality rate is as high as 90%.So the abdominal aortic aneurysm early intervention is very important.Currently,the main way in the treatment of abdominal aortic aneurysm includes two kinds,one is the traditional open repair(OR),the other is endovascular aneurysm repair(EVAR).Now in the clinical work,cavity repair because of its small trauma,postoperative low short-term complications of unique advantages,has been as the primary treatment of abdominal aortic aneurysm.After abdominal aortic aneurysm cavity repair surgery remains the corresponding complications,the most common is the internal leakage.At present,ultrasonic with its non-invasive,no radiation,the advantages of the repeatability is good,as the first choice for the postoperative abdominal aortic aneurysm cavity repair methods,in the detection of abdominal aortic aneurysm cavity repair has good detection rate in the postoperative complications.ObjectiveThis topic aims to evaluate ultrasound diagnostic clinical value of leak complications after abdominal aortic aneurysm surgery,at the same time compared with CTA,to provide reliable reference basis for clinical early diagnosis and early intervention.Methods42 cases of patients with infrarenal abdominal aortic aneurysm diagnosed by vascular surgery from January 2016 to February 2017 in the fifth affiliated hospital of Zhengzhou university were collected.After abdominal aortic aneurysm cavity repair treatment,collect related information and record each patient gender,age,etc.To determine whether patients with abdominal aortic aneurysm cavity repair postoperative existence of leakage,if there is a leak,determine the parts of the leakage and the leakage points,each patient in postoperative follow-up 1 month,3 months,6 months of the ultrasonic test results,comparative analysis at the same time corresponding CTA test results,calculation for two kinds of inspection within the abdominal aortic aneurysm cavity repair postoperative leakage detection rate.Results42 cases of patients with infrarenal abdominal aortic aneurysm cavity were examed after endovascular aneurysm repairing in 1 month.Ultrasonic detection of endoleak in 13 cases,including 5 cases of type?(1 case of type?a,4 cases of type?b),6 cases of type,2 cases of type? ?;The results of CTA show that 10 cases of endoleak were examed,including 5 cases of type?(1 case of type?a,4 cases of type?b),4 cases of type?,1 case of type?.Postoperative 3 months review,? type of leakage and type ? endoleak after surgical intervention,? type of leakage has not been treated.The examination show that the results of ultrasound and CTA examination are same that after surgical intervention of ? type of leakage and ? type of leakage are disappearing.Without processing ? type of leakage,leakage is still within only 2 cases of type ?,ultrasound and CTA are the existence of the endoleak detection and classification,but the diameter of abdominal aortic aneurysm were not obviously increase,so we do not make further processing;6 months review of ultrasound and CTA,inspection results and the postoperative 3 months review result is the same.Ultrasound for abdominal aortic aneurysm with endovascular aneurysm repair,detection rate of endoleak is 30.9%,CTA for abdominal aortic aneurysm with endovascular aneurysm repair,detection rate of endoleak is 23.8%.McNemar test to calculate the p> 0.05,two check ways no significant statistical difference(p= 0.375),the ? is 0.703,two kinds of examination are highly consistent.ConclusionsDetection effect of ultrasound in the abdominal aortic aneurysm with endovascular aneurysm repair of postoperative endoleak is consistent with the CTA,for repairing postoperative patients with abdominal aortic aneurysm provides another follow-up examination means.In addition,two kinds of examination can complement each other,mutual authentication.Ultrasound provides reliable diagnostic basis for clinical.There is great value in clinical application.
