| ObjectiveThe aim of the study was to provide baseline data for the prevention,diagnosis and treatment of cardiovascular and cerebrovascular disease in the region,which have shown that the condition of carotid artery intima-media thicknessand plaq-ue in the middle-elderly population by ultrasound detection in Minhang District, Shanghai.Methods1.A total3081patients in Min hang District examed carotid artery from April2012to January2013in our hospital, age from40to96years old,male1485, female1596. All patients were divided into five groups,group A:age from40-49years old,total352patients,with196males and156females,group B:age from50-59years old,total572patients,with274males and298females,group C:ag-e from60-69years old,total707patients,with314males and393females,grou-p D:age from70-79years old,total894patients,with.425males and469fem-ales,group E:age from80-96years old,total556patients,with281males and275females.2.A11the examinanion were conducted by well-qualify ultrusound doctors,using color doppler ultrasonic instrument.Data of intima-media thickness(IMT) and ath-erosclerotic plaque(including the characitistic,size,score,etc.)was the major collec-tion.3.Uniform training of specialized personnel to conduct the survey to obtain rele-vant information.Results1. This research included3081people(male1485, female1596), males of aver-age age was66.2±14.2years, and female of average age was66.8±13.2ye-ars. A group of CCA left and right sides of IMT was (0.67±0.17)mm and (0.64±0.15)mm,BIF left and right sides of IMT was (0.86±0.18)mm and (0.84±0.18)mm,ICA left and right sides of IMT was(0.61±0.13)mm and (0.61±0.13) mm,B group of CCA left and right sides of IMT was(0.75±0.15)mm and (0.75±0.15)mm,BIF left and right sides of IMT was (0.97±0.15)mm and (0.97±0.15)mm,ICA left and right sides of IMT was (0.67±0.13)mm and (0.68±0.13) mm,C group of CCA left and right sides of IMT was (0.81±0.15)mm and (0.86±0.16)mm,BIF left andright sides of IMT was (1.02±0.14)mm and (1.05±0.14)mm,ICA left and right sides of IMT was (0.69±0.12)mm and (0.69±0.12) mm,D group of CCA left andright sides of IMT was (0.87±0.15)mm and (0.86±0.15)mm,BIF left and right sides of IMT was (1.07±0.12)mm and (1.05±0.14)mm,ICA left and right sides ofIMT was (0.72±0.12)mm and (0.72±0.12) mm,E group of CCA left and right sides of IMT was (0.92±0.17)mm and (0.89±0.16)mm, BIF left and right side of IMT was (1.11±0.13)mm and (1.08±0.10)mm,ICA left and right sides of IMT was (0.73±0.12)mm and (0.73±0. 12)mm;CCA left and right sides of IMT was (0.84±0.24)mm and (0.80±0.21) mm, ICA left and right sides of IMT was (0.70±0.21)mm and (0.69±0.13)m m, BIF left and right sides of IMT was (1.02±0.24)mm and (1.01±0.20)mm;I-n men and women of left CCA IMT was (0.85±0.17)mm and (0.79±0.17)m m,In men and women of right CCA IMT was (0.83±0.18)mm and (0.78±0.21) mm,In men and women of left BIF IMT was (1.05±0.17)mm and (0.99±0.15) mm,In men and women of right BIF IMTwas1.03±0.15mm and0.98±0.15mm, In men and women of left ICA IMT was (0.72±0.14)mm and (0.67±0.12)mm, In men and women of right ICA IMT was (0.72±0.14)mm and (0.67±0.11) mm;Carotid artery IMT compared between the left and right carotidartery,gende-r and ages, all have statistically significant (P<0.05), and increasewith age,car-otid artery IMT showed a gradually increasing trend;between the IMT in same side of common carotid artery, internal carotid artery and carotid bifurcation,all have statistically significant (P<0.05).2. Left and right sides of common carotid artery IMT and age of correlation coefficient of r was(0.457,0.495)(P<0.05),Left and right sides of Carotid bifurc-ation IMT and age of correlation coefficient of r was (0.317,0.327)(P<0.05).L-eft and right sides of Internal carotid artery IMT and age of correlation coeffic-ient of r was(0.286,0.290)(P<0.05);A group of CCA left and right sides of I-MT and age correlation coefficient of r and correlation coefficient of b was(0.273,0.003) and (0.322,0.011)(P<0.05),B group of CCA left and right sides of IMT and age correlation coefficient of r and correlation coefficient of b was (0.113,0.006) and (0.096,0.016)(P<0.05),C group of CCA left and right sides of IMT and age correlation coefficient of r and correlation coefficient of b w-as (0.007,0.001)and (0.054,0.003)(P<0.05), D group of CCA left and right si-des of IMT and age correlation coefficient of r and correlation coefficient of b was (0.110,0.005) and (0.044,0.002)(P<0.05),E group of CCA left and right sides of IMT and age correlation coefficient of r and correlation coefficient of b was (0.05,0.004) and(0.016,0.001)(P<0.05);Common carotid artery,Internal c-arotid artery and carotidartery bifurcation of IMT with age was a linear positi-ve correlation,among linear relationship of the most closely was CCA IMT and age,Common carotid artery of IMT thickening degree from50to60years old than other ages.3. Carotid artery total plaque score was (3.05±3.69)mm, male (2.33±3.09)mm, female (3.83±4.12)mm,the left carotid artery plaque score was (1.61±2.01)mm, the right’carotid artery plaque score was (1.46±1.97)mm, A group of carotid a rtery plaque total score was (0.56±1.51)mm, B group of carotid artery plaque total score was (1.53±2.65)mm, C group of carotid artery total plaque score was (2.59±3.28)mm, D group of carotid artery plaque total score was (3.99±3.83)mm,E group of carotid artery plaque total score was (5.31±4.09)mm.Carot-id artery plaque total score compared between the left and right carotid artery, gender and ages, all have statistically significant (P<0.05),Carotid artery plaque score showed a gradually increasing trend with increasing age. 4. Carotid artery IMT thickening detection rate was76.8%, male82.6%,femal71.3%, the left carotid artery IMT thickening detection rate was70%, the right carotid artery thickening IMT detection rate was69.9%.A group of carotid arte-ry IMT thickening detection rate was4.1%,B group of carotid artery IMT thi-ckening detection rate was15.9%,C group of carotid artery IMT thickening det-ection rate was23.3%,D group of carotid artery IMT thickening detection rate was34.0%. E group of carotid artery IMT thickening detection rate was22.7%;Carotid artery AS plaque detection rate was62.1%, male68%, femal56.5%,the left carotid artery AS plaque detection rate was52.1%, the right caroti-d artery AS plaque detection rate was49.5%,A group of AS plaque detection rate of3.5%,B group of AS plaque detection rate was11.7%,C group of AS p-laque detection rate was22.3%,D group of AS plaque detection rate was36.8%,and the E group of AS plaque detection rate was25.7%.AS plaque of CC-A detection rate was18.8%, male63.1%,femal36.9%; AS plaque of ICA det-ection rate was20.4%, male68.1%,female31.9%:AS plaque of BIF detectio-n rate was60.8%, male53.1%,femal46.9%.Carotid artery IMT thickening and plaque detection rate compared between the left and right carotid artery,gender and ages,all have statistically significant (P<0.05);Carotid artery IMT thickenin-g and plaque detection rate in four groups of A, B, C, D compared between the gender,have statistically significant (P<0.05),E group of carotidartery IMT t-hickening and plaque detection rate compared between the gender,have no stati-stically significant (P>0.05);Carotid artery IMT thickening and plaque detection rate was gradually increasing trend with increasing age.but more than80year-s old,carotid artery IMT thickening and plaque detection rate have decreased,m-ale of IMT thickening and plaque detection rate than female;Compared between the difference of CCA,ICA and BIF IMT thickening and plaque detection rate have statistically significant (P<0.05),which BIF of the highest detection rate;Di-fferent parts of the ipsilateral carotid artery IMT thickening and plaque detecti-on rate compared between the gender, have statistically significant (P<0.05).5. The carotid artery stenosis detection rate was8%,mild stenosis was52%,mo-derate stenosis was26.3%, severe stenosis was14.6%and occlusion was7%, A group of carotid stenosis detection rate was1.6%,the detection rate of B gr-oup was carotid stenosis5.7%,carotid stenosis detection rate of C wasl6.2%,ca-rotid stenosis detection rate of D group was35.6%,carotid stenosis detection ra-te of E group was41.0%; Stable plaque detection rate was45.6%(including f-ibrous plaque39.4%, calcified plaque6.2%).unstable plaque detection rate was54.7%(soft plaque1.3%, mixed plaque53.4%).Carotid artery stenosis detection rate compared between the ages, have statistically significant(P<0.05);the four different nature of the plaque were compared between the ages,all have statisti-cally significant(P<0.05).Carotid artery stenosis was gradually increasing trend with increasing age.With increasing age,the carotid artery stenosis detection rate was gradually growing trend;With increasing of age the different nature plaque detection rate was gradually increasing trend in less than80years old,but mor-e than80years old the different nature plaque detection rate have decreased;C-arotid artery stenosis is main in mild and moderate stenosis;the different natur-e of carotid artery plaque detection rates following:mixed plaque>fibrous plaqu e>calcified plaque>soft plaque. Conclusions1.The degree of carotid atherosclerosis of the old in Minhang district was positively coefficient with the age,which the age increasing,the carotid atherosclerosis tended to progress.The male carotid atherosclerosis was serious than female carotid atherosclerosis.The detree of the carotid IMT thickening in50-59years old group was significance higher than other groups.The detectable rate of local carotid artery IMT thickness and plaque was relatively higher than other district,especially at the carotid artery bifurcation of the carotid IMT thickening atherosclerosis.2.The early pathological change of carotid artery detected by ultrasound,the car-otid atherosclerosis could be detected early and more precise,futher more,can pr-ovide base-line data and objective evidence to the clinicians for the early diag-nosis and prevention,the incidence of cardiovascular and cerebrovascular disea-se in Minhang distict would be effectively reduces,which makes great signifien-ce in improving the life quality of lacal population. |
