Objective: to analyze the intracranial and extracranial atherosclerotic plaque distribution by carotid duplex ultrasound and TCD.Methods: 290 cases with stroke in Lanzhou University Second Hospital from June 2015 to April 2016,and make a selection(meet the following criteria: 1 patients with cerebral infarction(confirmed by CT/MRI),including acute and chronic cerebral infarction;2 clinical diagnosis occurred within the past 3 months,the onset of TIA patients(more than 1 episodes(CT/MRI)showed that there was no ischemic lesions)exclusion criteria: 1.patients with cerebral hemorrhage caused by cerebral trauma;2.cervical artery dissection,Takayasu's arteritis etc.the intracranial vascular stenosis or occlusion;3.patent foramen ovale(PFO)caused by stroke;4.moyamoya disease;more than and 5.sclerosis;6.mitochondrial encephalomyopathy;7.the incomplete information;analysis of the value of ultrasound diagnosis of intracranial vascular.Results: in the carotid artery color Doppler ultrasound,13.6% cases of extracranial segment were not abnormal,while those with extracranial atherosclerosis accounted for 88.5%.About 74.2% of the carotid artery atherosclerotic plaque,the plaque in patients with 30% single,70% the number of plaques in patients with less than 2,ultrasound examination revealed 236 vessels have different degrees of stenosis,the internal carotid artery(including bulbar and proximal internal carotid artery stenosis)the proportion of the largest(43.6%),vertebral artery and the subclavian artery is second,and the proportion is close to(respectively 21.7% and 20%).The degree of extracranial artery stenosis in 50%,accounted for 42.8% of the total number of stenosis and extracranial artery stenosis is more than 70%,accounting for 26.1% of the total number of stenosis.In more than 50% stenosis and aged 70%,still in the internal carotid artery(including carotid bulb and proximal internal carotid artery)stenosis of vertebral artery is the largest proportion,followed by.But in more than 50% stenosis and over 70% vessels,the internal carotid artery and vertebral artery stenosis proportion increased,while the common carotid artery and subclavian artery stenosis proportion decreased(P < 0.0001).Conclusion: carotid artery color Doppler ultrasound can be used in the diagnosis of the stenosis and stenosis rate of different parts.And through the statistical study,hyperlipidemia,diabetes,hypertension,family history of cerebrovascular stenoses for ischemic stroke common risk factors,and only a minority of patients with ischemic stroke with atrial fibrillation,statistical significance is not obvious,so it is not considered.Objective Carotid atherosclerotic plaque is regarded as an important risk factor of ischemic cerebrovascular disease.This study is to analyze the factors influencing the number of carotid atherosclerotic plaque in patients with ischemic stroke.Methods 194 patients with ischemic stroke admitted to the department of neurology of Second Hospital Affiliated to Lanzhou University from December 2015 to December 2016 were selected.neck vascular color Doppler ultrasound were performed,statistics for each case of internal carotid artery in patients with plaque and plaque number according to the number of different,divided into single,double and multiple three groups,The patients' sex,age,hypertension,hyperlipemia,diabetes and history of ischemic stroke were recorded.Multivariate Logistic regression analysis was used to analyze the influencing factors for plaques.Results: Multivariate Logistic regression analysis showed that gender,hypertension,hyperlipidemia,history of cerebral infarction was risk factor for single carotid artery plaque,hyperlipidemia,diabetes mellitus was risk factor for double carotid artery plaque,gender,diabetes,history of cerebral infarction was risk factor for multiple carotid artery plaque.Conclusion: Different number of plaques may have common risk factors and different number of plaque morphology have their own risk factors.
