Correlation Between Serum Thyroid Antibodies,TSH And Ultrasonography And Differentiated Thyroid Cancer

Objective: To explore the relationship between serum thyroid antibodies,TSH and ultrasonography and thyroid malignancy,and to construct a diagnostic prediction model to provide a basis for differential diagnosis of benign and malignant thyroid nodules in clinical work.Methods: This study collected general information,laboratory and ultrasonography data of the first hospitalized patients with thyroid nodules in the First People's Hospital of Changde City from November 2017 to March 2019.The differences in age,gender,serum thyroid antibodies,TSH and other clinical variables between the DTC group and the benign nodule group were compared.And then,this study used multivariate logistic regression analysis to identify independent predictors of thyroid cancer and to construct a diagnostic predictive model for differentiated thyroid cancer..RESULTS: 1.A total of 337 subjects were enrolled in this study.The age,nodule diameter,ultrasound grade,Tg Ab positive rate,TPOAb positive rate and TSH elevation in DTC patients were significantly different from those in benign nodules(P<0.05).2.Multiple regression analysis showed ultrasound grading(OR: 4.76,95% CI: 3.28-6.90),positive Tg Ab(OR: 5.30,95% CI: 2.28-12.35),positive TPOAb(OR: 3.07,95% CI: 1.39-6.78),abnormal TSH(OR: 2.32,95% CI: 1.21-4.45),age <50 years(OR: 3.45,95% CI: 1.72-6.67)were independent risk factors for DTC.3.The predictive model for diagnosis of DTC had an AUC of 0.922(0.894,0.950),a sensitivity of 90.8%(84.7-95.0%),and a specificity of 80.10%(73.8-85.5%).The NRI value of the predictive model relative to ultrasound grading diagnosis of DTC was 8.0%(0.3-13.3%).Conclusions: Age <50 years old,positive Tg Ab,positive TPOAb,abnormal TSH and ultrasound grading(?4a)were independent risk factors for DTC.Diagnostic prediction model has significantly enhanced the diagnostic value of ultrasonography for DTC.
