Objective To compare the diagnostic value of256-slice CT angiography in head andneck with carotid vascular ultrasonography and DSA in extracranial carotid artery diseaseof patients with acute cerebral infarction, then offer the reliable basis for etiologicdiagnosis and treatment.Methods There are258hospitalized patients with acute cerebral infarction wereselected from January2012to June2013in the affiliated hospital of Hebei UnitedUniversity department of Neurology. All the patients were examined by256-slice CTangiography in head and neck and carotid vascular ultrasonography. It is to analysis thesensitivity, specificity and consistency in diagnosis of extracranial carotid artery diseaseand the different detection rate of plaque between the two methods.30patients (180vessels)of them were examined by DSA at the same time, and were analysed thesensitivity, specificity and consistency in diagnosis of extracranial carotid artery diseasebetween256-slice CT angiography in head and neck and DSA through comparing theresults.Results1The compliance rate of256-slice CT angiography in head and neck andcarotid vascular ultrasonography in the diagnosis of extracranial carotid artery stenosiswas73.5%(1138/1548), and they have lower degree of consistency in mild and moderatestenosis (Kappa testing, K=0.01/0.05) and high in sever stenosis and blocking (Kappatesting, K=0.34/0.47).2The detective rate in mild and moderate stenosis and plaquedetection in carotid and external carotid artery by256-slice CT angiography in head andneck is higher than carotid vascular ultrasonography.3The compliance rate of256-sliceCT angiography in head and neck and DSA in the diagnosis of extracranial carotid arterystenosis was82.8%(149/180) and they have high strength consistency in the diagnosis ofdifferent degrees of stenosis, espcially in the sever stenosis (Kappa testing, K=0.75).Conclusions1256-slice CT angiography in head and neck is superior carotid vascularultrasonography in diagnosis of extracranial carotid artery stenosis and plaque in patientswith acute cerebral infarction.2256-slice CT angiography in head and neck has a highcorrelation with DSA in the diagnosis of extracranial carotid artery stenosis, which caneven replate DSA in a certain extent.
