| Objective The purpose of this study was to investigate the Characteristics of atherosclerosis severity degree in extracranial carotid arteries (ECCA) and the change of cerebrovascular hemodynamic index (CVDI) in patients with ischemic cerebrovascular disease of carotid artery system (ICVDCAS),and to explore the correlation between carotid artery intima-media thickness (IMT)and CVDI and the diagnositic value in ICVDCAS. Methods 102 hospitalized patients with ICVDCAS and 31 control subjects without ischemic cerebrovascular disease comparable with age and sex were detected by colour-Doppler ultrasound and CVDI .Results (1) The prevalence of plaque in ECCA of patients with ICVDCAS and control group were 71.6% and 45.16% respectively. The respective prevalence of severe lumen stenosis in ECCA(>50%) in patients and control group were 20.6% and 3.2%.Moreover,soft-plaque and mixed-plaques could be more frequently encountered in carotid artery of higher-grade stenosis .(2) After multivariate analysis ,hypertention(HT),diabetes mellitus(DM) and smoking were independent predictors of the extracranial carotid atherosclerosis (P <0.01) and the prevalence of severe lumen stenosis (>0%)was 30.8%.(3) There is a high prevalence of plaque rupture in ECCA and clinical symptomes in patients differ from each other.(4) The prevalence of plaque in ECCA of patients with ICVDCAS was more 60%. Moreover,soft-plaque and mixed-plaques could be more frequently encountered in carotid artery of higher-grade stenosis.(5) Hemodynamic parameters such as EDV,RI,PI decreased in patient with Binswanger's disease and multiple lacunar infact(MLI)(P<0.05). (6) The CVDI had remarkable changes in patients with ICVDCAS ,including Vmin,DR,R and Zc(P<0.05).However,there were no significant difference between the lesion side of brain hemisphere and the other (P>0.05).(7) IMT were negatively correlation with Vmean,Vmin,Vmax,Qmean and Qmax. Conclusion The atherosclerosis degree in ECCA and the change of CVDI in patients with ICVDVAS is more severe than those of control group. The measurement of atherosclerosis degree and CVDI should be of important clinical value in for diagnosing and choosing treatment methods。... |
