Health is one the primary needs for survival, and is the basis for allthe other rights to development. Medical issue of the poor has become anew focus for global health services in21st century. Since the governmentstarted the urban medical assistance (UMA) in2005, it has been developedrapidly. Shanghai government responded to the call from centralgovernment positively and has made a big progress in the establishment ofUMA. However, there are still many bottlenecks, such as the conflictbetween huge health demand and limited UMA funs, high precipitationand low utilization of funds. As the fund is a basic element for operation ofthe urban medical assistance system (UMAs), it is necessary to study onthe financial demanding for UMAurgently.This paper reviews the equity theory of medical services, publicadministration theory, the theory of medical services efficiency, theprinciples of risk stratification and allocation principles, etc, and concludesthat it is reasonable to take the target group’s demands for medical service into account in the first place. This study is aimed to building up acalculation model on the basis of reasonably define the scope of UMAfunds, which is made of five steps: firstly, to calculate the sum of families’economic risks for health care of the poor; secondly, to measure theindividual’s economic risks for medical services; thirdly, the financialdemand of UMA under ideal model; fourthly, the financial demand ofUMA under realistic model; fifthly, to calculate the financial demand ofUMA dynamically.After reviewing the UMAs and basic medical insuranceof Shanghai, the data of the target group’s medical services fee are appliedto the model.Through theory analysis and model testing, the method of measuringthe financial demand for UMA is scientific, and the measuring model isapplicable and operable.
