| ObjectivesTo evaluate the clinical efficacy of 125 I seeds implantation combines with chemotherapy of gemcitabine plus cisplatin to treat pancreatic cancer. MethodsClinical data of 51 pancreatic cancer patients(35 males and 16 females,aging from 38 to 74 years old) in the first affiliated hospital of Zhengzhou University from December 2010 to December 2013 were retrospectively analyzed, both of the patients have been taken the CT guaide biopsy,and all of them have been comfirmed pancreatic cancer.Tumor in the head of the pancreatic were 31 cases,tumor in the body of the pancreatic were 12 cases and tumor in the tail of the pancreatic were 8 cases.All the patients were divided into two groups,the observation group treated by 125 I seeds implantation under CT-guided, combined chemotherapy with gemcitabine plus cisplatin.The control group treated by chemotherapy with gemcitabine plus cisplatin. The two groups were followed up for 1 to 36 months. Comparing two the groups of patients cancer treatment effect,toxic reaction, clinical benefit response and survival analysis. Results The effective(CR+PR) rate of observation group was 70.4%, the effective rate of control group was 37.5%, there is significant difference between the two groups. In the observation group,the class â…¢,â…£ leucocytes decreasing rate is 37.0%,the class â…¢,â…£ hemoglobin decreasing rate is 22.2%,the class â…¢,â…£ thrombocyte decreasing rate is 25.9%, by contrast in the control group there were 29.2%ã€20.8% and 25.0%. The class â…¢, â…£ nausea and vomiting rates were 22.2% and 20.8%. The toxic reaction between observation group and control group were no significant differences. The clinical reflects of benefit effective rate was 55.6% in the observation group,and the control group clinical reflects of benefit effective rate was 25.0%, there were significant difference between the two groups. Two months after treatment the observation group CEA and CA19-9 levels were significant reduced than the control group. The median survival time of the observation group was 15.32 months, the survival rates of 12 and 24 months were 55.2% and 6.9%; the median survival time of the control group was 10.94 months, the survival rates of 12 and 24 months were 22.2% and 0, there were significant difference between the two groups. The TNM stage, tumor size, KPS scores and CA19-9 were major independent risk factors of survival. Conclusions1.125 I seeds implantation combined with chemotherapy of gemcitabine plus cisplatin in treatment of pancreatic cancer had an affirmative clinical efficacy, could effective improve the quality of life and to prolong survival of the patients.2. The CEA and CA19-9 levels were significantly reduced after treatment.3. The survival of patients with advanced pancreatic cancer could effectively extend.4. The TNM stage, tumor size, CA19-9 after treatment and KPS scores were independent factors in survival in patients with advanced pancreatic cancer, in which the TNM stage, tumor size, KPS scores and CA19-9 were major independent risk factors of survival. |
