Objective:Our study aims to comparatively analyze the short-term effects of laparoscopic radical resection of rectal carcinoma with traditional laparotomy surgery.Setting:Department of Gastrointestinal Surgery, Shandong Provincial Hospital affiliated to Shandong University (including central and eastern compound)Methods:1. Research object In January 2014 to January 2015 in our department for treatment of 76 cases of laparoscopic radical resection of rectal carcinoma patients, and 84 patients with traditional laparotomy surgery respectively. All the patients have the indication of Miles operation and no dysfunction of other organs. The 76 patients and 84 patients were divided into group A and group B. Compare and analyse the clinical data of the two groups of the patients.2. Research observation index General index:age, sex, other complications. Surgical index:type of resection, amount of bleeding during the operation, autonomic nerve damage and preoperative radiotherapy.Pathological index:tumor location, tumor size, tumor height and pathological classification (pTNM).3. Statistical analysis Statistical software SPSS 19.0 was used for data analysis. P<0.05 presents significant difference.Results:1. There is no difference in statistics in the tissue pathology, age, gender, tumor stage, tumor location of the two groups of patients.2. The total operation time of Laparoscopic group and laparotomy group was separately (168 ± 46) min and (162 ± 40) min, the amount of bleeding during operation of the two groups of patients were (164 ± 38) ml and (266 ± 54) ml, the hospitalization time of two groups of patients after operation were (7.2 ± 2.2) d, (14.1 ± 6.1) d, recovery time of intestinal of two groups of patients after operation were (2.25 ± 0.75)d、(3.82 ± 1.82) d, the total cost of two groups of patients are (53.6 ± 8) and (39.5 ± 7) thousand yuan. It showed that there were significant statistical differences between the data of the two groups above (p<0.05).3. Through the comparison of the recovering situation of the two groups, including the exhaust time, eating time, the number of incision infection, postoperative hospitalization time after operation of the two groups patients respectively. The recovery of the laparoscopic group was better than the laparotomy group after operation.Conclusion:For the patients who have the indications of Miles operation, laparoscopic treatment have the significant advantages over the traditional laparotomy surgery, and the treatment effect of the operation of the laparoscopic operation was in the same way as the laparotomy surgery. The patients recover better than that treated in the laparotomy surgery. So it is feasible and safe to do the laparoscopic operation.
