Expression And Clinical Significance Of Toll-like Receptors 4, IL-7 And IL-10 In The Salivary Glands Of Patients With Sj?gren’s Syndrome

Objective:To study on the expression of TLR4, IL-7, IL-10 in labial salivary glands the patients with Sjogren’s syndrome, and analyze the correlation between each factor and other laboratory indicators, and explore the effect of TLR4 and other related factors in the incidence and development of Sjogren syndrome, to provide a new thinking for the treatment and diagnosis of Sjogren’s syndrome.Methods:74 cases patients with Sjogren’s syndrome were as the study grop, which had treated in the first affiliated hospital of Bengbu medical college during 2010.02 to2013.10, while 32 cases patients as the control group, treated in the same period. Then obtain the labial gland biopsy of all patients, all the pathological sections were received quantitative and qualitative analysis by elivision immunohistochemical staining method and artificial counting method, as follow:(1) Study about the positive expression rates of TLR4, IL-7 and IL-10 in the labial glands of the patients with Sjogrensyndrome, and compared the color intensity and staining scores of all TLR4, IL-7 and IL-10 in labial gland.(2) Study on the correlation between the expression of TLR4, IL-7, IL-10 and the correlation between TLR4, IL-7, IL-10 and other laboratory indicators.(3) Compared the positive expression between primary Sjogren’s syndrome and secondary Sjogren’s syndrome. Finally, all the data were statistic by SPSS13.0 software package. The measurement data was expressed as(±s), and the comparison between two groups by t test, the count data was expressed by n(%), and the comparison between two groups by x2 test. Multiple comparison of much groups by SNK-q test in AVONA. The test level between two groups was 0.05,and could corrected accord the number of groups. P<0.05 showed the difference had a statistically significant, and P<0.01 showed the difference had a statistically significant obviously.Results(1) The positive expression rate of TLR4 in the study group in the labial gland of study group and control group was 79.72%, 53.12%, the difference was statistically significant((x2=5.471,P=0.031)). The color intensity scores of TLR4 of the study group and control group were 1.27±0.29 and 0.46±0.51, respectively(P=0.013), the and color staining score of study and control group were 3.24±3.07 and 0.58±0.66, the difference had a statistically significant(P<0.001).(2)The positive expression of IL-7 in the labial gland of study group and control group were 85.14%, and 21.87%, and the difference had a statistically significant(x2=17.539,P=0.000). The color intensity scores of study group and study group were 1.31±0.27 and 0.26±0.11, and the difference had a statistically significant(P=0.021). The staining scores of study group and control group were3.37±1.61 and 0.54±0.67, the difference had a statistically significant(P=0.014).(3) The positive expression rate of IL-10 in the labial gland the study group was of the patients was 82.43%, while the control group was 34.28%, and the difference was statistically significant between the two groups(x2=10.204,P=0.013). The color intensity score of I-10 of study group and control group were1.49±0.22 and 0.31±0.17,and the difference had a statistically significant(P=0.019). The color stained score of I-10 of study group and control group were3.41±1.04 and 0.59±0.61, and the difference had a statistically significant(P=0.024).(4) The positive expression rate of TLR4, IL-7, IL-10 were positively correlated with each other, and had statistical significance(P<0.05).(5) The positive expression rate of TLR4, IL-7 and IL-10 of primary Sjogren’s syndrome and secondary Sjogren’s syndrome in labial gland was higher than the control group, the difference was statistically significant(P<0.05). But there had no a difference between primary Sjogren’s syndrome and secondary Sjogren’s syndrome(P>0.05).(6) The positive expression of IL-7 and IL-10 was positively correlated with ESR, which had statistical significance(r=0.539,P=0.003;r=0.494,P=0.013). The positive expression of TLR4 was positively correlated with Ig M( r=0.358,P=0.03), Ig A( r=0.461,P=0.012), ESR(r=0.303,P=0.032) and other immune parameters, the difference was statistically significant(P<0.05).Conclusions(1) The expression level of TLR4 、IL-7 and IL-10 are significantly higher than those in normal patients, but there has no difference between primary Sjogren’s syndrome and secondary Sjogren’s syndrome, suggested the TLR4 signal transduction pathway and its related cytokines may be involved in the pathogenesis of Sjogren’s syndrome.(2) The expression of TLR4、IL-7、IL-10 are positively correlated with ESR、Ig M and Ig A, it suggest there is a correlation between the expression of TLR4 and IL7 and the activity of Sjogren’s syndrome, IL10, can be used as a reference index for clinical evaluation of Sjogren’s syndrome.(3) There may has a correlation between inflammatory or immune factors with Sjogren’s syndrome, In view of the TLR4 signal transduction pathway,targeted therapy may be beneficial to control the development of dryness syndrome.
