Background:Cholesterol ester transfer protein (CETP),the key protein in the cholesterol ester reverse transport (RCT) system which play an important role in lipoprotein metabolism. There were numerous studies have been reported about the CETP gene polymorphisms determinants for plasma CETP activity, concentration and high level of HDL-c, what may be closely related with the occurrence of coronary heart disease (CHD).Type 2 diabetes mellitus (T2DM) and hypertension patients often accompanied with dyslipidemia and hyperuricemia, above of that are all independent of atherosclerosis (AS) and CHD risk factors.The relationship between CETP gene polymorphism and type 2 diabetes and high blood pressure research there have been some reports, but has yet to achieve consistent conclusions.Objective:Through the analysis of the guangxi zhuang autonomous region healthy people, people with type 2 diabetes, type 2 diabetes and high blood pressure, cholesterol ester transfer protein (CETP) Taq IB gene polymorphism, investigate Taq IB and type 2 diabetes and high blood pressure, the relationship between discusses Taq IB influence on blood lipid and uric acid levels, as well as a risk factor for type 2 diabetes and hypertension.Objects and methods:Application of polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) method, the guangxi zhuang autonomous region, the crowd of 72 cases (group A control group) and 222 cases of type 2 diabetes (group B-type 2 diabetic foot, including 111 cases of type 2 diabetes mellitus without hypertension, type 2 diabetes and hypertension,111 cases) of the CETP Taq IB site of gene analysis and type 2 diabetes and hypertension risk factors analysis, explore the CETP Taq IB the genotype and allele frequency distribution of the genotype and uric acid (Ua), total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-c), low density lipoprotein cholesterol (LDL-c), the relationship between uric acid (Ua), and other indicators, understand the CETP Taq IB polymorphism in guangxi region likely role in type 2 diabetes and high blood pressure, and the risk factors of type 2 diabetes and high blood pressure.Result:1. General situation and biochemical examination level:type 2 diabetes mellitus group (group B) BMI{(24.97+3.18) kg/m2}is higher than the control group (group A){ (24.07±1.98) kg/m2}, P=0.000;Type 2 diabetes group of systolic blood pressure (SBP){(129.09+20.29) mmHg} is higher than the control group{ (123.46-20.18) mmHg}, P=0.001;Type 2 diabetes group of diastolic blood pressure (DBP){(79.09+13.03) mmHg} levels higher than the control group (group A){(77.00-13.67) mmHg}, P= 0.018;Type 2 diabetes mellitus group TC{ (5.40±1.29) mmol/l}, TG{1.43 (0.34-19.46) mmol/l}, LDL-c{(3.36 ± 3.72) mmol/1} were higher than the control group [(5.31 ■ 0.88) mmol/l,{1.20 (0.42-5.30) mmol/l}, (3.24±0.73) mmol/l], P was 0.001,0.020,0.009, respectively.The incidences of low hypercholesterolemia to type 2 diabetes (17.6%) than the control group (5.6%), P=0.012.2. Genotyping:(1) the CETP Taq IB genotype B1B2 based on hybrid mode in both group is given priority to, the frequency is greater than 70%, B1B1 (about 19%), B2B2 (about 9%);Allele B1 (about 55%), B2 (about 45%) roughly equal frequency.(2) the CETP Taq IB genotype frequency and allele frequency distribution in no difference was found between the two groups, P was 0.975 and 0.984.(3) after sex, whether it is in the control group, type 2 diabetes mellitus group, or merger of the crowd, three kinds of genotype frequency and allele frequency distribution, there was no statistically significant difference between men and women, P (genotype frequency) were 0.432,0.635,0.682, P (allele frequency) were 0.603,0.646,0.511;Comparison between groups, control group and type 2 diabetes group male crowd CETP TaqIB no statistically significant differences in genotype frequency distribution, P=0.612, the control group and type 2 diabetes group of female population CETP TaqIB no statistically significant differences in genotype frequency distribution, P= 0.613.3. Genotypes with BMI relationship is further divided into two groups:(1) the BMI< 25 kg/m2 (normal body weight)and BMI (overweight)≥25 kg/m2, the result shows that type 2 diabetes group of overweight rate (33.8%) than the control group (16.7%), P=0.006.(2) whether the control group, type 2 diabetes, or merger of the crowd, normal weight and overweight in three kinds of genotype frequency and allele frequency distribution difference is not significant, P (genotype frequency) were 0.762,0.496,0.891, P (allele frequency) were 0.601,0.701,0.832; according to the comparison between the control group and type 2 diabetes group,of normal weight people CETP TaqIB no statistically significant differences in genotype frequency distribution, P=0.682, two groups of overweight people CETP TaqIB also no statistically significant differences in genotype frequency distribution, P=0.614.4. The CETP gene TaqIB polymorphism analysis of relation:(1) after merge all the object of study, three kinds of genotypes in BMI, SBP, DBP, FBG,2 HPBG, Ua, TC, TG, HDL-c, there was no statistically significant difference level of LDL-c, PBMI=0.254, PSBP=0.897, PDBP=0.462, PFBG=0.075, P2hPBG =0.083, PUa=0.352, PTC=0.596, PTG=0.174, PHDL-c=0.496, PLDL-c= 0.919;(2) only in people with type 2 diabetes, the three genotypes in FBG, TG52hPBG, HbA1C, the difference was statistically significant, PFBG=0.035, P2hPBG=0.040, PHbAlc=0.013, PTG=0.046, show the B2B2 FBG{6.57 (5.35— 18.85) mmol/l},2 HPBG{10.23 (7.39—32.70) mmol/l}below B1B1{{8.91 (5.53—25.07) mmol/l、11.40 (7.13—31.34) mmol/l}and B1B2{7.66 (5.88— 30.12) mmol/l,11.45 (7.28—36.25) mmol/l}, B1B1 TG level{1.57 (0.45-9.06) mmol/1} above B1B2{1.50 (0.34~19.46) mmol/1} and B2B2{(1.22 (0.41-7.26) mmol/1}.5. A risk factor for type 2 diabetes and hypertension analysis:the 222 cases of patients with type 2 diabetes, according to the history of high blood pressure have divided into no hypertension group (111 cases) and hypertension group (111 cases), (1) of the hypertension group BMI{(24.93 ± 3.55) kg/m2} is higher than without hypertension group{(22.95±3.15) kg/m2}, P=0.000, the hypertension group HbAlC{9.53 (6.00-19.20)%}higher than that of without hypertension group{9.46 (4.90-17.40)%}, P= 0.035;(2) in the two groups in the CETP TaqIB differences in genotype and allele frequency distribution has no statistical significance, P were 0.881,0.672;(3) there is high blood pressure as the dependent variable (without hypertension is 0, have high blood pressure is 1), with gender (men is 2 to 1, women), age, BMI, smoking history (no smoking history is 0, has a history of smoking to 1), diabetes duration, fasting plasma glucose, postprandial blood sugar, glycosylated hemoglobin, uric acid, cholesterol, triglycerides, high-density lipoprotein cholesterol, low density lipoprotein cholesterol, CETP TaqIB genotype (B1B1 is 1, B1B2 to 2, B2B2 for 3) as the independent variable in Logistic regression, the results show that age (P = 0.000), and BMI (P=0.000), TG (P=0.034) into the equation, the CETP TaqIB polymorphism gene into the equation.Conclusion:1. Cholesterol ester transfer protein (CETP) Taq IB genotypes and alleles in healthy people and people with type 2 diabetes, no obvious difference was found between the frequency distribution, the CETP TaqIB genotypes and alleles in men and women, the frequency distribution of normal weight and overweight people have no difference.2. Among type 2 diabetes, the CETP TaqI B2B2 gene carriers of low blood sugar levels than B1B1, B1B2, B1B1 gene carriers of high TG levels are B1B2, B2B2.3. Merge in people with type 2 diabetes patients with high blood pressure is easy to merge obese patients with high blood pressure;Logistic regression results suggest age, BMI, TG levels increase 1 unit, type 2 diabetes and high blood pressure risk increased 0.072,0.239,0.354 times, respectively, the CETP TaqIB gene polymorphism and type 2 diabetes and the occurrence of hypertension has no obvious correlation.
