ObjectiveTo observe the clinical effect of combining traditional chinese and western medicine treatment of severe acute pancreatitis,and investigate the curative effect of Tongfu Qingyi soup in treatment of severe acute pancreatitis on relieving the role of disease progression and prognosis of outcome.MethodsA total of 40 patients who conform to the diagnosis standard were chosen in the Second Affiliated Hospital of Nan Yang Medical College from October 2011 to December 2014, informed consent were signed by patients or family member before treatment, then were divided into two groups, conventional western medicine treatment, such as fasting, gastrointestinal decompression, re-hydration and maintaining circulation stabilization, preventing shock, inhibiting pancreatic secretion (photostatting) and so on, timely surgical intervention with operation indications during the treatment; The treatment group had 22 patients, given conventional treatment of western medicine combined with Tongfu Qingyi soup treated by nasal feeding-oral sequential, combined with the application of traditional chinese medicine enema, slag abdominal wet compress; The control group had 18 patients, given conventional western medicine treatment. To observe two groups on the relief time of abdominal symptoms and signs, laboratory indicators of recovery, the first defecation time, complication rate, transfer rate of operation, organ failure rate, the cure rate, mortality rate and the mean hospitalized days.Resultsl.The two groups have no statistically significant difference (P> 0.05) on abdominal pain and distension, peritoneal irritation sign positive number, blood and urine amylase value, white blood cell count, abnormal blood sugar level.Compared with control group, the group of Combining traditional Chinese and western medical treatment, the time of stomachache relief (3.75±0.75 d VS 4.77±1.09 d, t=3.11, P=0.02), the first bowel movement time (2.41±0.73d VS 3.14±0.74 d, t=3.11, P=0.04), blood amylase returned to normal time (4.00±2.80 d VS 6.00±2.44 d, t=2.729, P=0.01), urine amylase returned to normal time(6.27±1.83 VS 9.61±3.46 d, t=3.261,P=0.04), white blood cell count back to normal time (4.59±1.25 d VS 7.27±3.30 d, t=3.261, P=0.04), abnormal blood sugar back to normal time(7.66±2.73 d VS 11.33±3.16 d, t=2.316, P=0.03).And the two groups on the relief time of abdominal symptoms and signs, laboratory indicators of recovery and the first defecation time have statistically significant difference (P< 0.05)。2.Compared with control group, the group of combining traditional Chinese and western medical treatment,surgical operation rate of the acute stage [0%(0/22) VS 27.8%(5/18), X2=4.701, P=0.03], the rate of complications in treatment group (six cases of pancreas and pancreatic abscess, three cases of pancreatic pseudocyst,0 case of gastrointestinal fistula,0 case of retroperitoneal bleeding) [27.3%(6/22) VS 50.0%(9/18), P<0.05], and in the control group(4 cases of pancreas and pancreatic abscess,3 cases of pancreatic pseudocyst, lcase of gastrointestinal fistula,1 case of retroperitoneal bleeding), organ failure rate [0%(00/22)) VS 22.2%(4/18), X2=5.126, P=0.024]. 3.Compared with control group, the group of Combining traditional Chinese and western medical treatment have a good result:the cure rate (77.3% VS 61.1%, P<0.05) the effective rate (90.9% VS 72.2%, X2=4.048, P=0.044), and the mean hospital stay (19.45±3.7 d VS 29.83±9.3 d,t=4.77, P=0.01)Conclusions1. Combining traditional Chinese and western medical treatment of SAP have a good and remarkable effect on improving abdominal symptoms and signs, blood and urine amylase, and abnormal blood sugar recovery.2. Combining traditional Chinese and western medical treatment of SAP obviously decrease the operation transfer rate and the rate of pancreas local complications (around the pancreas and pancreatic abscess and pancreatic pseudocyst), gastrointestinal fistula and retroperitoneal bleeding, which reduce hospitalized time and are worthy of clinical popularization and application for Tongfu Qingyi soup in the process of the treatment of SAP.
