Objective:Analyse the survival time,clinical impact factors and complication of patients with advanced pancreatic cancer,evaluate the effect and safety of lower power,heat accumulated HIFU treat to these patients. Compare the survival time of Low Power,Heat Accumulated HIFU and traditional method HIFU Methods,evaluate the curative effects of two methods.Methods:We analyzed121cases (from January2008to December2013) Treated in the second Affiliated Hospital of Zhejiang University School of Medicine.They were treated by HIFU and they are patients with stage3or4pancreatic cancer.Traditional methods including38cases and heat accumulation method including83cases.We use the FEP-BY02system to perform HIFU treatment. Obverse the change of biochemical factors before and after HIFU treatment, complication.Obverse and analysis survival time of two methods.Results:In this study, among the83patients treated by heat accumulation method HIFU,10.53%(8/76) was found Serum Amylase rise,5.56%(4/72)was found Total Bilirubin rise,20.78%(16/77) was found CRP rise,1.26(1/79)was found Total Bilirubin leukocytosis rise after the treatment;30.12%(25/83) feel pain,10.84%(9/83) present with transient fever,2.41%(2/83) present with blister at the skin of ultrasound channel,4.82%(4/83) present with subcutaneous induration at the skin of ultrasound channel,4.82%(4/83) feel weak,27.71%(23/83) feel nausea after the treatment.Heat accumulation method survival HIFU, Phase3/4MST are12vs.9months, OS:six months85.6%vs.60.7%,1year43.6%vs.28.0%,2years6.8%vs.7.8%;Among the patients with pancreatic cancer initial diagnosed as stage III and did not undergo any chemotherapy, heat accumulation method and traditional method of HIFU, MST is10vs.5months (p=0.021),6-month and1-year OS was84.6%vs.38.6%and35.2%vs0%;among the patients with pancreatic cancer initial diagnosed as stage IV and did not undergo any chemotherapy, heat accumulation method and traditional method of HIFU, MST were7vs3months (p=0.003),6months and1year OS was53.8%vs13.3%and33.7%vs0%; among the patients with pancreatic cancer initial diagnosed as stage III and undergo gemcitabine chemotherapy, heat accumulation method and traditional method of HIFU, MST was12vs15. months (p=0.388), no significant difference; among the patients with pancreatic cancer initial diagnosed as stage IV and undergo gemcitabine chemotherapy, theat accumulation method and traditional method of HIFU,MST is9vs5months (p=0.186),no significant difference. Conclusion:As a kind of noninvasive treatment, Low Power,heat accumulated HIFU is safe.It has few side effects, but has certain effect, therefore, this technology has broad prospects in advanced pancreatic cancer therapy.Low Power,heat accumulated HIFU has more survival benefit,especially for Patients with stage III or IV pancreatic cancer who have a poor constitution and unable to tolerate chemotherapy.
