Acupuncture For Insomnia Accompanying With Anxiety:a Randomized Placebo Control Trial

Objective:To observe the clinical curative effect of acupuncture and sham acupuncture for the insomnia along with anxiety, and compare their difference. Findings would provide scientific evidence for acupuncture treatment of insomnia, anxiety and improve life quality of poor sleepers.Methods:67insomnia sufferers with anxiety were randomly divided into two groups:33in treatment group and34in control group. Those in the treatment group received acupuncture, while control group adopted sham acupuncture. And both interventions were conducted by using the Park Sham Placebo Acupuncture Device (PSD). Outcomes were Pittsburgh sleep quality index (PSQI), Zung self rating anxiety scale (SAS), steady-state visual evoked potential (SSVEP) and needle scale index changes. Data of two groups were compared before and after intervention.Results:(1) Baseline data:Gender, age, education backgroud, TCM symptoms, course of insomnia, anxiety state, Pittsburgh sleep duration index (PSQI) and grade of insomnia, anxiety, the anxiety rating scale (SAS), grading, steady-state visual evoked potential (SSVEP), showed no statistical significance (P>0.05).(2) Inter group comparison:the effective rate was statistically significant (P<0.05); assessments of the PSQI, SAS, SSVEP brain potential value and needle scale after the treatment, on the first and second follow-up check revealed statistical significance (P<0.05). Sharp pain score of needling were almost the same in two groups (P>0.05).(3) Inner group comparison:PSQI, SAS, norm and the brain potential value comparison of SSVEP showed statistically significant before and after the treatment in acupuncture group(P<0.05), but no difference in control group(P>0.05); As for sleep efficiency, the left temporal lobe and parietal lobe potential value comparison were statistically significant (P<0.05). Assessments on the first follow-up of PSQI scores, sleep quality and sleep duration revealed statistically significant (P<0.05) in acupuncture group, however sleep duration, sleep efficiency, sleep disorder, daytime dysfunction and SAS showed no statistical significance (P>0.05). While in control group, PSQI scores, sleep quality, sleep time, sleep duration, sleep efficiency and daytime dysfunction failed to show any statistical significance (P>0.05) in the same period; and disorders and SAS had improved with statistical significance (P<0.05). Acupuncture group and a follow-up after the second round of follow-up and treatment of the PSQI, SAS, norm, with statistical significance (*P<0.05). In control group, sleep quality and sleep time of the second follow-up checking and sleep quality, sleep time, sleep time, sleep efficiency, sleep disorder of the first follow-up time point revealed no statistical significance (P>0.05); But PSQI scores, sleep efficiency, sleep disorder and daytime dysfunction after treatment, PSQI scores and daytime dysfunction of the first follow-up checking, as well as SAS score after treatment and first follow-up checking all demostrated statistically significance (P<0.05).Conclusion:1. Acupuncture therapy improved the clinical symptoms and emotion status. It revealed a short-term curative effect, but failed to maintain that in a long run. Conversely, sham acupuncture didn’t show any improvements in insomnia or anxiety management.2. Acupuncture reduced the visual evoked potential value in steady-state area in the brain of patients, especially in the frontal lobe and occipital lobe. As for sham acupuncture, it reduced the potential value of part of the parietal lobe.3. The severer symptoms were, the more serious anxiety would be. Acupuncture is effective means to improve insomnia associated with anxiety and the quality of life, which is worthy of attention and promotion.
