The Clinical Therapeutic Evaluation On The Transurethral Bi-polar Vaporization Of The Prostate Among High Risk Patients

Purpose:To evaluate the advantages and disadvantages of transurethral mono- and bi-polar vaporization of the prostate(TUVP vs TUBVP) through comparing the physiological parameters between the two procedures for treating benign prostatic hyperplasia (BPH). Method:Sixty-eight patients with BPH were grouped in two: Group I, the whole conditions were better than another group, received TUVP.Group II, whose conditions were worse or with more than one high risk factors of surgery, received TUBVP.Tested and measured the relating physiological parameters pre-, in- and post-operation. Result:Group II were older (t=2.683, P=0.009) and the rate of diabetes mellitus was larger(X2=6.346, P=0.012), with higher blood sugar pre-operation (t=2.244, P=0.028) .There were no differences on the operating time consuming, blood loss and the volume of irrigation during operation (t value is 1.043 0.177 1.061 respectively and Pwas0.301 0.860 0.293) .Group II had less irrigation (t=3.219, P=0.002) and hospitalization time post-operation (t=4.634, P=0.000) .The pain of urethra post-operation in Group II was rare, milder and transient, lasting less than 3 days; but Group I mostly were more common, obvious and longer, lasting for 3~10days, what's more, hematuria was more serious, lasting for 1~2 weeks.The bi-polar electrode was more expensive than mono-electrode by 2000 yuan, but other cost of treatment was less 800 yuan. Conclusion:TUBVP is more effective and safe with extended indications and less complications; the patients suffering from BPH with high risk factors were better adapted to TUBVP, but that cost more than TUVP.
