Clinical Application Of Laparoscopic-endoscopic Cooperative In Gastric Stromal Tumor Surgery

Background：Gastrointestinal stromal tumor is a mesenchymal tissuesources tumors with potentially malignant tendency.Surgical resection isthe preferred method of treatment of localized gastric stromaltumors,laparoscopic-endoscopic cooperative surgery is a new method oftreatment carried out in recent years, but its operation safety andthoroughness remains controversial.In this study, to explore the safetyand effectiveness of laparoscopic-endoscopic cooperative surgery（LECS）of gastric stromal tumors,we compared clinical data oflaparoscopic-endoscopic cooperative treatment of patients with clinicaldata of open treatment of patients.Methods：From september2008to september2011，56patients withgastic GIST in LECS group （30cases）and in open group（26cases）were analyzed retrospectively.operativetime,blood Loss,postoperativeexhaust time,hospital stay,postoperative Complications,postoperativepathological data and postoperat-Ive pathological data and ostoperativerecurrence rate were Compared between the two groups.Results: The operation of56patients were successful, there were30cases in the LECS group and23cases in the open group.There were nosignificant differences between the two groups with respect to sex，age，tumor size，mitotic rate,risk stratifi-cation and tumor resection method.The LECS group mean age（57.03±8.31）yesrs old，the average tumor diameter（4.65±1.76）cm.The open group mean age（55.03±8.02）yesrsold，the average tumor diameter（5.03±1.90）cm.the operative time，blood loss,postoperative exhaust time and hospital stay in LECS groupwas less than those of the open group (96.93±21.08vs127.00±19.18min，46.27±11.93vs109.53±11.66ml，2.22±0.60vs3.35±0.47d，8.73±2.52vs12.00±2.27d).There was significant difference between thetwo groups(P<0.05).howev-er,the operative time,the rate of postoperativecomplications,the recurrence rate had no significant difference(3.3％vs7.6％，3.3%vs3.8%，P>0.05).Conclusions:1.There was no significant difference in operative time,postoperative complications,recurrence rate between the two groups,butlaparoscopic-endoscopic cooperative surgery group has advantages inintraoperative blood loss, exhaust time, hospital stay.2.The laparoscopic-endoscopic cooperative surgery is feasible andsafe，and to better effect than Laparotomy.
