| Objective: To observe the effect of invigorating the spleen and kidney, supplem enting qi and activating blood circulation method on early diabet ic(Diabetic Nephr opathy, DN) and the clinical efficacy of renal protective effect of DN model rats, and t o explore its mechanism of action.Methods: 1.60 patients with early diagnosis of diabetic nep hropat hy patien t s, randomly divided into experimental group and control group with 30 cases in eac h group, the experimental group was given the traditional Chinese medicine of invigo rating the spleen and kidney, Yiqi Huoxue Decoction(decoction 300 ml, sooner or lat er, clothing, daily 1 agent), the control group was given Valsartan Capsules(80mg/, 1 times daily), treatment 8 weeks, observed before and after treatment in two groups of patients with clinical symptoms, fasting plasma glucose(FPG), 2h postprandial blood g lucose(PBG), glycosylated hemoglobin(Hb A1c),urinary albumin excretion rate(UA E), blood pressure(BP), serum creatinine(SCr), blood urea nitrogen n(BUN), cholester ol(TC), triglyceride(TG), beta 2 microglobulin(beta 2-MG) etc.. 2 of the 60 male SD rats were selected from 48 only uses the unilateral removal of the kidneycaused by i ntraperitoneal injection of STZ DN model, were randomly divided into model group,c ontrol group(valsartan group), low dose of Chinese medicine group, high dose of Ch inese medicine group. The remaining 12 right kidney fa lse resection an d intraperitone al injection ofan equal amount of sodium citrate buffer as sham operation group. Mode l 2D, sham operation group, model group were given physiological saline 4ml orally, t he control group was given valsartan 28.8mg/kg D Ig, the low-dose group of Chines e medicine and Chinese medicine high dose group were given medicine formula gran ule 1.5g/kg D(2 times adult dose), 3g/kg D(4 times adult dosagee) by gavage. Respectively, before treatment, rats in general, 24 h urinary protein, urinary albumin, urine were o bserved 8 weeks after treatment with β2 microglobulin. Fasting blood glucose, blood u rea nitrogen, crea tinine, choles terol, triglyceride,endothelin-1, immunohistochemical d etection of TGF-β1 expres sion in renal tissue.Results: 1 after 8 weeks of treatment in experimental group of TCM syndromes in patients with index decreased obviously(P<0.05), UAE, beta 2-MG(P<0.05) decre ased significantly, BUN,Scr, TC, TG showed no significant change(P>0.05), Jianpi Bus hen yiqi huo xue recipe through improving the clinical symptoms, reduce urinary prot ein, improve the early treatment of early renal plays the role of DN. 2 spleen and kid ney, invigorating qi and activating blood circulation can significantly reduce the D N rat model of 24 h urine protein, urine m ALB, beta 2-MG, blood FPG, BUN, Scr, T C, TG, ET-1(P<0.05) and decreased TGF- beta 1(P<0.05) expression in renal tissue, im prove the renal tissue lesions, show that the spleen and kidney, Yiqi Huoxue recipe ca n reduce the blood sugar of DN model rats, improve renal function, decrease the excr etion of urine protein, decrease the level of serum ET-1 and TGF-β1 expression in th e kidney, delaying the process of DN.Conclusion: 1 the spleen and kidney, supplementing qi and activating blood circ ulation method can effectively reduce the blood glucose, blood lipid in patients with e arly diabetic nephropathy, urinary protein excretion, beta 2 microglobulin, and effectivel y improve the clinical symptoms and signs of patients. 2 spleen and kidney, suppleme nting qi and activating blood circulation method can effectively reduce the diabetic n ephropathy model rats blood glucose, triglyceride, cholesterol, urinary albumin, beta 2 microglobulin, 24 hours urine protein, serum urea nitrogen, serum creatinine. 3 splee n and kidney, supplementing qi and activating blood circulation method can reduce th e level of serum endothelin, inhibition of TGF- beta 1 over expression, and protect th e kidney, delay the development of DN. 4 the mechanism of invigorating the spleen and kidney, supplementing qi and activating blood circulation method in treating diab etic nephropathy may improve renal blood rheology,inhibition of expression regulation of the microcirculation; pathogenic factor, inflammatory reaction; inhibition of ECM d eposition, reducing the permeability of GBM etc.. |
