Objective:By studying the expression of insulin-like growth factor 1(IGF-Ⅰ), insulin-like growth factor 1 receptor(IGF-ⅠR) and insulin-like growth factor binding protein 6(IGFBP-6) among the breast cancer, breast fibroadenoma and adjacent normal mucosa, we discuss the expression and correlation of IGF-Ⅰ, IGF-ⅠR and IGFBP-6 in different breast tissue. Analysing the relationship among the three factors in age, the size of nidus, histopathological subtypes, and clinical pathological indicators(e.g. with or without lymph node metastasis), so that we can diagnose breast cancer early and provide new targets for clinical treatment. So that, we can provide theoretical and experimental basis for recovery and prognosis of breast cancer.Method:80 cases of breast cancer(BC), 60 cases of breast fibroadenoma(BF) and 50 cases of normal breast tissue adjacenting to carcinoma(NB) were Collected. All specimens were confirmed by pathological diagnosis in affiliated hospital of chengde medical college from April,2013 to February,2014. Using the immune histochemical method to detect the expression of IGF-Ⅰ, IGF-ⅠR and IGFBP-6 protein, and using reverse transcription polymerase chain reaction(rt-pcr) to detect the expression quantity of IGF-Ⅰ m RNA, IGF-ⅠR m RNA and IGFBP-6 m RNA. We analyzed experimental and clinical pathological data and assessed the relationship.Results:1 The expression of IGF-Ⅰ, IGF-ⅠR, IGFBP-6, IGF-Ⅰ m RNA、IGF-ⅠR m RNA and IGFBP-6 m RNA in breast cancer, breast fibro adenoma and normal breast tissue adjacent to carcinoma.1.1 The expression of IGF-Ⅰ、IGF-ⅠR and IGFBP-6 in BC, BF, NBThe expression of IGF-Ⅰand IGF-ⅠR in BC was significantly higher than those of in BF(P<0.05) and NB(P<0.05). And the expression of IGF-Ⅰand IGF-ⅠR in BF was significantly higher than those of in NB(P<0.05);The expression of IGFBP-6 in BC was significantly lower than those of in BF(P<0.05) and NB(P<0.05). And the expression of IGFBP-6 in BF was significantly lower than those of in NB(P<0.05).1.2 The expression of IGF-Ⅰ m RNA、IGF-ⅠR m RNA and IGFBP-6 m RNA in BC, BF, NBThe expression of IGF-Ⅰ,IGF-ⅠR and IGFBP-6 m RNA in BC was significantly higher than those of in BF(P<0.05) and NB(P<0.05). And the expression of IGF-Ⅰ,IGF-ⅠR and IGFBP-6 m RNA in BF was significantly higher than those of in NB(P<0.05).2 The relationship between IGF-Ⅰ、IGF-ⅠR IGFBP-6、IGF-Ⅰ m RNA, IGF-ⅠR m RNA and IGFBP-6 m RNA and clinical pathological characteristic of breast cancer.2.1 The relationship between IGF-Ⅰ, IGF-Ⅰ m RNA and pathological characteristic of BCThe relationship between the expression of IGF-Ⅰ and IGF-Ⅰ m RNA of breast cancer were accordance with the clinical pathological indicators. The positive expression of IGF-Ⅰ and IGF-Ⅰ m RNA of primary tumors larger(>2.5cm) in breast cancer was higher than smaller(≤2.5 cm)(P<0.05);With lymph node metastasis group were significantly higher than those without lymph node metastasis group(P<0.05).Credits in histopathologic type III level is significantly higher than the class II significantly higher than the level I(P<0.05).The expression of IGF-Ⅰ and IGF-Ⅰ m RNA of breast cancer in the age group(≤49,>49) had no significant statistical difference(P>0.05).2.2 The relationship between IGF- Ⅰ R, IGF- Ⅰ R m RNA and pathological characteristic of BCThe relationship between the expression of IGF- Ⅰ R and IGF- Ⅰ R 7 m RNA of breast cancer were accordance with the clinical pathological indicators. The positive expression of IGF-ⅠR and IGF-ⅠR m RNA of primary tumors larger(>2.5cm) in breast cancer was higher than smaller(≤2.5 cm)(P<0.05);With lymph node metastasis group were significantly higher than those without lymph node metastasis group(P<0.05).Credits in histopathologic type III level is significantly higher than the class II significantly higher than the level I(P<0.05).The expression of IGF-ⅠR and IGF-ⅠR m RNA of breast cancer in the age group(≤49,>49) had no significant statistical difference(P>0.05).2.3 The relationship between IGFBP-6, IGFBP-6 m RNA and pathological characteristic of BCThe relationship between the expression of IGFBP-6 and IGFBP-6 m RNA of breast cancer were not accordance with the clinical pathological indicators. By protein expression in IGFBP-6:The positive expression of IGFBP-6 of primary tumors larger(>2.5cm) in breast cancer was lower than smaller(≤2.5 cm)(P<0.05);With lymph node metastasis group were significantly lower than those without lymph node metastasis group(P<0.05),Credits in histopathologic type III level is significantly lower than the class II significantly lower than the level I(P<0.05);By protein expression in IGFBP-6 m RNA:The positive expression of IGFBP-6 m RNA of primary tumors larger(>2.5cm) in breast cancer was higher than smaller(≤2.5 cm)(P<0.05);With lymph node metastasis group were significantly higher than those without lymph node metastasis group(P<0.05), Credits in histopathologic type III level is significantly lower than the class II significantly higher than the level I(P<0.05).The expression of IGF-Ⅰ and IGF-Ⅰ m RNA of breast cancer in the age group(≤49,>49) had no significant statistical difference(P>0.05).3 The relationship among IGF-Ⅰ、IGF-ⅠR and IGFBP-6 in BC3.1 The relationship between IGF-Ⅰ and IGF-ⅠRThe positive relationship could be seen between IGF-Ⅰ and IGF-ⅠR(r=0.576, P<0.01).3.2 The relationship between IGF-Ⅰ and IGFBP-6The negative relationship could be seen between IGF- Ⅰ and IGFBP-6(r=-0.603, P<0.01).3.3 The relationship between IGF-ⅠR and IGFBP-6The negative relationship could be seen between IGF- Ⅰ R and IGFBP-6(r=-0.632, P<0.01).Result:1 The level of IGF-Ⅰand IGF-ⅠR expression in the mammary gland hyperplasia, breast fibro adenoma and breast cancer tissues is gradually strengthened, prompts that the high expression of IGF-Ⅰ and IGF-ⅠR in the process and occurrence of breast cancer play an important role, so we can speculate IGF-Ⅰ and IGF-ⅠR can predict breast cancer as an important factor. And IGFBP-6 expression in mammary gland hyperplasia, breast fibro adenoma and breast cancer reduce gradually, so the high expression of IGFBP-6 may inhibit the development of breast cancer.2 The significant difference expression of IGF-Ⅰ and IGF-ⅠR relate to the original focus size and the presence of lymph node metastasis, it can prompt IGF-Ⅰand IGF-ⅠR play a facilitating role in growth, invasion and distant metastasis of breast cancer, and can predict breast cancer metastasis and prognosis. The IGFBP-6 high expression relate to breast cancer primary focal size and the presence of lymph node metastasis, so we conjecture IGFBP-6 high expression can inhibits the invasion and metastasis of breast cancer.3 IGF-Ⅰin breast cancer have a significant positive correlation with IGF-Ⅰ R,in the breast cancer tissue which IGF- Ⅰ is high expression the expression of IGF-ⅠR is high too, it shows a combination of synergy effect in the development of breast cancer. in the breast cancer tissues which IGFBP-6 express highly IGF-Ⅰ and IGF-ⅠR express rather low, we can presumably the IGFBP-6 high expression can inhibit the expression of IGF-Ⅰ and IGF-ⅠR, thus inhibit the development of breast cancer.So we can predicted to reduce or block the expression of IGF-Ⅰand IGF-ⅠR, or enhance the expression of IGFBP-6 by some way, may help prevent breast cancer or reduce the effect of breast cancer metastasis.4 That we inhibit or block the IGF-Ⅰ and IGF-ⅠR or improve IGFBP-6 effects on the expression of someone factor by some way, may effect other factors’ excessive expression and invasion of breast cancer, thereby reducing the risk of breast cancer metastasis.
