Study About Clinical Features Of 58 Male Breast Cancer

Objective: Through the review of case study, sum up the general case,clinical characteristics and pathological features,explore the current situation of the disease treatment and analysis the relevant factors affecting the 58 MBC trated by the General Hospital of Ningxia Medical University over pass twenty years.To provide theoretical reference to clinical treatment and prognosis estimation.Methods: 58 MBC cases that were histologically diagnosed and treated by the General Hospital of Ningxia Medical University from January 1994 to December 2013 were collected. To understand the age, the proportion, and morbidity trend of MBC,summarize the clinical characteristics of MBC and understand its current situation.Meanwhile, the relevant factors affecting the MBC prognosis are analysised through the follow-up visit in telephone or re-examination.The duration of case study starts from when the MBC was diagnosed and finished in December 2013. Survival time is from diagnosis to the death of the patient. It records the time of last follow-up visit if patients with MBC were still alive. At the same time, the case SPSS17.0 software was used for statistical analysis. t test or analysis of variance were utilized to measure data.χ2test was used to count data. Univariate analysis was performed by Kaplan-Meier method. Survival rates were compared using log-rank test, the prognosis multivariate analysis was completed by Cox proportional hazards model, P<0.05 was determined to be a statistically significant difference.Results: 1. 6846 cases of BC patients were treated the General Hospital of Ningxia Medical University between 1994 and 2013,male cases were 0.85%(58/6846),famale cases were 99.15%(6788/6846).The ratio of male and female is 1:117. The overall number of cases showed a rising trend,1994, 1995, 1996,1997 and 1998 the number of incidence ratio was 1.72%(1/58), 1.72%(1/58),3.45%(2/58), 1.72%(1/58) and 3.45%(2/58);2009, 2010, 2011, 2012 and 2013 were 5.17%(3/58), 8.62%(5/58), 8.62%(5/58), 6.89%(4/58) and 8.62%(5/58).2.The morbid age is from 22 to 80 years old. The median age is 61. The age of 20cases(34.48%) is between 60 and 69.The major initial sign of 58 MBC cases all were mass in breast,located in the left were 41.38%(24/58),right were 58.62%(34/58).MBC 58 cases in 56 cases can be classified according to tumor size T1(≤ 2cm), T2(2 ~ 5cm), T3(> 5cm), which accounted for 41.07%(23/56), 46.43%(26/56), 12.5%(7/56).The number of lymph node metastasis in patients with 1-3pieces accounted for 278.57%(16/56),4-9 pieces accounted for 21.43%(12/56),10 and above accounted for 8.93%( 5/56),0 accounted for 41.07%( 23/56),the remaining cases can not be estimated.I,II,III,,IVperiod patients were 20.69%(12/58),27.59%(16/58),25.86%(15/58),25.86%(15/58).Pathological type of infiltrating ductal carcinoma 89.66%(52/58), invasive lobular carcinoma accounts for 3.45%(2/58), accounting for 6.89% of other types(4/58).Pathological grade of I,II,III patients were 0.00%、85.71%(48/56)、14.29%(8/56),the remaining cases can not be estimated.18 cases of male breast cancer patients had a complete ER, PR, HER-2 and Ki-67 expression, based on their molecular typing,it showed that Luminal A subtype accounted for 27.78%( 5/18),Luminal B subtype 66.67%( 12/18),HER-2 over-expression subtype5.55%(1/18)and triple negative subtype 0%.58 MBC patients cases, 35 patientsunderwent a radical mastectomy, accounting for 60.34%, only 3 cases with simple tumor resection, accounting for 5.17%,received adjuvant chemotherapy in patients accounted for 37.93%(22/58); adjuvant radiotherapy patients accounted for13.79%(8/58), 17 cases of hormone receptor-positive patients, 12 patients received endocrine therapy, accounting for 70.59%(12/17).3.Follow-up time was4.0 to 122.0 months, lost five cases, dropout rate 8.62%(5/58).The median survival of MBC patients,The 1-,3-,5-,7-and10 year survival rates were94.84%,62.08%,42.36%,26.15%and 20.04%, during follow-up visit. Univariate analysis showed that: morbid age,tumor size, histological grade, regional lymph node metastasis,and staging are factors affecting MBC patients’ survival(P values were: 0.00,0.00,0.00,0.002). Ethnic, family history of cancer, primary site,histological type and pathological grading are not related with the prognosis of MBC patients(P values: 0.434,0.311,0.459,0.485,0.307). According COX multivariate analysis, lymph node metastasis, clinical stage is to determine the survival of patients with MBC independent prognostic factor(P values were0.007,0.007).Conclusion: 1. MBC is more rare, The General Hospital of Ningxia Medical University for nearly 20 years admitted to male breast cancer incidence is increasing year by year, especially in the past five years,the incidence of more common in the elderly, and increases with age, the incidence rate increasing.2.MBC common symptoms were the mass in the breast and tumor histological type more common in invasive ductal carcinoma, high grade, ER, PR and HER-2 positive rate low rate of positive molecular subtypes with Luminal B-type common. Lymph node metastasis and clinical stagewere independent prognostic factors affecting survival in MBC patients.
