Correlation Study Between Red Blood Cell Distribution Width, Serum Cystatin-C,carotid Intima-media Thickness And The Coronary Artery Lesion Severity

Objective:by comparing and observing the change of the three indicators of the red blood cell distribution width(RDW), serum cystatin-C(Cys-C), carotid intima-media thickness(IMT) between patients with coronary artery diseasea and the control group, to examine the relationship between RDW, Cys-C, IMT and the degree of coronary artery lesions and its clinical significance.Methods:choose 186 patients who were suspected coronary heart disease(CHD) and admitted to our department as the research object in the department of cardiology ward from June 2013 to September 2014, record completely the clinical baseline data(age, gender, smoking history, medical history, etc.) of all the patients. All patients in the hospital the next morning was fasting venous blood timely and sent to the hospital clinical laboratory, detection of blood routine and biochemical indexes of blood, and for all patients with carotid artery color doppler ultrasound. After being related check perfect and patients in stable condition, were performed coronary arteriography(the CAG) inspection, according to the result of coronary angiography, the coronary heart disease diagnosis standard, the research object was divided into 136 cases of coronary heart disease group and the control group of 50 cases. The general clinical data, red blood cell distribution width, related biochemical indexes and carotid intima-media thickness of the two groups were compared; again according to the result of the CAG calculation Gensini score, the CHD group was divided into Gensini1-30, 31 to 60 points, >60 points of three subgroups, and further compare RDW, Cys-C and IMT values level of more groups of patients; at the same time RDW, Cys-C and IMT values and Gensini score values of coronary artery lesions for correlation analysis.Results:⑴ Coronary heart disease group was compared with the control group in clinical data, smoking rates, the RDW, Cys-C and IMT levels of coronary heart disease group were significantly higher than that of control group, the difference was statistically significant(P <0.05).⑵ The comparison between different Gensini score group, with the increase of coronary artery lesions Gensini score values, the levels of RDW, Cys-C and IMT were increasing, the difference was statistically significant(P<0.05), only RDW values between Gensini1-30 group and Gensini 31-60 group was no statistically significant difference(P> 0.05).⑶ RDW, Cys-C and IMT values with Gensini score of correlation analysis showed that CHD group RDW, Cys-C and IMT values and Gensini score values of vascular with pathological changes are positively correlated{( r=0.369, P<0.001),( r=0.506, P<0.001),( r=0.719, P<0.001) }.Conclutions:these three indicators of RDW, Cys-C and IMT are positive correlation with the severity of coronary lesions, united measure the levels of RDW and Cys-C and IMT will have important clinical significance for the early prevention, diagnosis, treatment and prognosis assessment of coronary heart disease.
