Objective: To investigate the clinical features,diagnosis, treatment and prognosis of solid pseudopapillary tumor of the pancreas(SPTP).Method:The clinical data of SPTP cases that underwent surgical resection with a definitive histological diagnosis in Fujian Provincial Cancer Hospital from January 2005 to December 2014 were retrospectively analyzed.Result: There were 22 SPTP cases undergoing surgery in this period.Including 16 females and 6 males; they were ages 11 to 76 years(median,33 years).3 tumors were located in the head of the pancreas(13.6%), 2 in the neck(9.1%), 6 in the body(27.3%),and 11 in the tail(50%).There were 10 patients(45.5%) manifested as abdominal discomfort; 12 patients(54.5%) had no abdominal symptoms, in which 2 cases showed an abdominal bump at the beginning of physical examinations. All of the patients were performed surgical resection. 4 cases underwent pancreaticoduodenectomy, 3 cases were received local tumor resection, 2 cases received pancreatic segment resection,5 cases did spleen-preserving distal pancreatectomy, 8 cases received pancreatic body/tail resection and splenectomy.Average diameter of the tumors Was 9.0±4.1cm,6 lesions were solid,9 were solid-cystic,7 were cystic.The postoperative complication rate was 22.7%,4 cases developed pancreatic fistula and 1 case caused abdominal infection.22 cases were followed up, the mean follow-up time was 64 months and all of the cases were alive. During the follow-up period, we didn’t find any tumor recurrence or liver metastasis, or other distant metastasis.Conclusion: SPTP is a kind of uncommon neoplasm with low-grade malignancy and rarely metastasizes. It occurs mainly in young women lack of characteristic clinical manifestations and signs. CT has comparative characteristics. The diagnosis depends on pathology after the operation. Surgical resection is most favorable in the treatment of SPTP, which has excellent prognosis. The overall prognosis of SPTP is usually benign, the radical resection and negative margin are important to for a long survival.
