Objective: To investigate the clinical significance of serum IL-18 levels in acute organophosphorus pesticide poisoning patients. In this study, we observed the relationship between the levels of IL-18 and CRP、ALT、the severity of illness,and aimed to study the correlation between the levels of IL-18 and the severity of illness. And help to evaluate the early disease and prognosis.Methods:96 cases from January 2013 to January 2015 of acute organophosphorus pesticide poisoning patients were choosed,there were 70 patients in moderate poisoning group and 26 patients in severe poisoning group. At the same time, we choosed 22 healthy persons served as control group. All the acute organophosphorus pesticide poisoning patients’ blood collection were took 3 ml in the early morning, within the first day、the third day and the seventh day after acute organophosphorus pesticide poisoning, we separated serum within 3 hours, placing it at- 80 ℃ medical stored in fridge.Adopt double antibody sandwich ELISA method for the determination of serum level of IL- 18, kits were producted by the French Disclose company,strictly according to the manual operation, at 492 nm absorbance measurement values. At the same time taking 3 ml venous blood, separating serum, that day testing ALT 、 AST 、 TBIL in the Hitachi 7600-D type full automatic biochemical analyzer.Results: 1 The content of IL-18 in serum of patients with AOPP was decreased,but higher than that in control group. ALT, TBIL and IL-18 levels in serum of patients with AOPP were significantly higher than those in control group(P<0.01) in first, third and seventh day. The contents of TBIL, ALT and IL-18 in serum of patients with AOPP in first, third day were significantly higher than those in seventh day(P<0.01); The contents of IL-18, ALT and TBIL inserum of patients with AOPP were significantly higher than those in third day(P<0.01).2 The content of CRP in serum of patients with AOPP was decreased, but higher than that in control group. The CRP levels in serum of patients with AOPP were significantly higher than those in control group(P<0.01) in first,third and seventh days. The contents of CRP levels in serum of patients with AOPP in first, third day were significantly higher than those in seventh day(P<0.01); The contents of CRP levels in serum of patients with AOPP were significantly higher than those in third day(P<0.01).3 The contents of IL-18, CRP and ALT in serum of patients with moderate or severe poisoning were significantly higher than those in control group(P <0.01); The contents of IL-18, CRP and ALT in serum of patients with severe poisoning were significantly higher than those in moderate poisoning group(P<0.01). The change of IL-18 and CRP in serum of patients with AOPP was significantly positive(r=0.902,P<0.01). The change of IL-18 and ALT in serum of AOPP patients was significantly positive correlation(r=0.49, P <0.05); the change trend of IL-18 and APACHEII in serum of AOPP patients was significantly positive correlation(r=0.49, P< 0.05).4 The serum levels of IL-18 were significantly different between patients with cured patients and those who died, and the serum levels of IL-18 were significantly higher than those in the cured group(P< \0.05), the difference was statistically significant.Conclusion:1 Serum IL-18 levels were significantly elevated in patients with AOPP,and early detection of IL-18 levels had important reference value for predicting the degree of organic phosphorus poisoning, which can guide clinical medication and improve prognosis of patients.2 The ALT level and TBIL level In the serum of patients with AOPP were significantly increased, tips for AOPP patients accepted early treatment of liver has an important clinical significance, and can help to decrease hepatic impairment.3 The IL-18 level combiled with ALT level in patients with AOPP can be integrated to evaluate the severity of organic phosphorus poisoning and liver injury.
