Clinical Evaluation Of The Effect Of Jiang Tang Bao Shen Decoction 1 On Early Diabetic Nephropathy

Objective to explore the treatment of early diabetic nephropathy deficiency of both qi and Yin and blood stasis by observing Jiang Tang Bao Shen Decoction I (降糖保肾1号方)in the treatment of early diabetic nephropathy (Qi and yin deficiency and blood stasis type) in patients with TCM syndrome score, fasting blood glucose,2-hour postprandial blood glucose, serum creatinine, blood urea nitrogen, urinary albumin concentration, urinary microalbumin/ creatinine ratio, blood pressure and low density lipoprotein cholesterol (LDL-C). Our objective was to improve the clinical curative effect of early diabetic nephropathy with Qi and yin deficiency and blood stasis endogenous, effectively control and delay the progression of the disease.Methods a total of 60 patients met the inclusion criteria, were randomly divided into treatment group (30 cases) and control group (30 cases). Patients in the two groups were given diabetic diet and exercise control, antidiabetic, antihypertensive and lipid-lowering etc.The treatment group in the conventional western medicine treatment on the basis of plus Jiang Tang Bao Shen Decoction I(降糖保肾1号方)treatment, daily 1 agent, points 2 times oral; control group in conventional western medicine therapy plus placebo treatment, daily 1 agent, points 2 times oral, course of treatment in the two groups are 60 days. TCM syndrome score, fasting blood glucose,2-hour postprandial blood glucose, serum creatinine, blood urea nitrogen, urinary albumin concentration, urinary microalbumin/creatinine ratio, blood pressure and low density lipoprotein cholesterol (LDL-C) were conducted before and after the treatment, and indicators of therapeutic effects were recorded and scored. After the end of the study, the two groups of patients with TCM syndrome score, laboratory tests and other clinical data were collected, sorted out. analyzed, summarized, and thus concluded that the study.Results There are one case of drop out in control group, the actual completion of 59 cases. including 30 cases of treatment group.29 cases in the control group. The results showed that:1. the overall efficacy of the two groups were significantly different (P<0.05), with a total efficacy of 83.3% in treatment group,68.9% in control group.2. After treatment, the TCM symptom scores of both of treatment group and control group were significantly better than before (P<0.05或P<0.01).In intergroup comparison, treatment group is superior to control group(P<0.05)in terms of aggregate score of symptoms and symptom scores such as drinking, more frequent urination, frequent urination at night, stool unhappy, upset, insomnia, fatigue, limb numbness, soreness and weakness of waist and knees, the more frequent urination, frequent urination at night, uncomfortable stool, fatigue, five clinical symptom improvement is particularly significant (P<0.01).3. After treatment, the urinary albumin concentration, urinary microalbumin/creatinine ratio were significantly better than before (P<0.05),especially in treatment group(P<0.01). In intergroup comparison, treatment group is superior to control group (P<0.01) in urinary albumin concentration, urinary microalbumin/creatinine ratio.4. Comparison of two groups of patients before and after treatment, blood pressure,blood glucose and blood lipid levels were significantly decreased (P<0.01); in intergroup comparison, there is no different of blood pressure.blood glucose and blood lipid between two group after the treatment (P>0.05).5. there is no different of serum creatinine, blood urea nitrogen and blood pressure between two group after the treatment (P>0.05)Conclusions Jiang Tang Bao Shen Decoction I(降糖保肾1号方)Combined with western medicine treatment has obvious advantages in the total clinical effect, can better improve the patient’s clinical symptoms and signs, reduce thd urinary albumin concentration, urinary microalbumin creatinine ratio, fasting blood glucose.2-hour postprandial, having a certain effect on blood pressure,blood glucose, and blood lipid. Jiang Tang Bao Shen Decoctaion I(降糖保肾1号方)has better efficacy and less adverse than pure western treatment. Jiang Tang Bao Shen Decoction I is better use in the treatment of early diabetic nephropathy. It can slow down the process of diabetic nephropathy, help to control the illness and improve the overall clinical symptoms and improve the quality of life of patients.
