Effect Of Glutamine Enriched Bile Reinfusion On Serum CRP And Immunologic Function In Patients With Benign Obstructive Jaundice After Operation

Objective: To study Effect of glutamineenriched bile reinfusion on serum CRP and immunologic function in patients with benign obstructive jaundice after operation.Methods: This study included 44 benign obstructive jaundice patients who needed biliary tract external drainage in our Hospital From October 2013 to September 2015.Acording to the prospective、randomized method, the patients were randomly divided into glutamine enriched bile reinfusion group(A group), bile reinfusion group( B).Serum CRP, immunoglobulin Ig A 、 Ig M 、 Ig G and T lymphocyte subsets(CD4+,CD8+,CD4+/CD8+) were tested at the day before operation and at the day 1,7 after operation. we should enjoin patient semirecumbent position or inclined 30 degrees before bile reinfusion,thus we could avoid bile back transmission time,bile reflux caused discomfort, at the same time we closely observed the patient’s condition and truthfully recorded the A,B two groups of patients with complications of infection, infection complications include: abdominal infection, incision infection, pulmonary infection. All data were inputed into computer, through SPASS16.0 software for statistical analysis. All data were analyzed by using chi-square test, analysis of variance(ANOVA) for repeated measurement and independent samples t-test.Results: One day after the operation, the immune indexes of the glutamineenriched bile reinfusion group(A group), bile reinfusion group(B group) were lower than that on the day before operation(P<0.05), The Serum CRP of the two groups on the first day after operation were higher than that on the day before operation(P<0.05),the surgey aggravated immune function and inflammation. Nine days after two different treatments, the two groups of various immune indexes were recoverd and Serum CRP value were decreased obciously. The level of Ig G、CD4+、CD4+/ CD8+at the seventh day after operation were ascend in the glutamineenriched bile reinfusion than bile reinfusion(P < 0.05); The Serum CRP was lower in the glutamineenriched bile reinfusion than bile reinfusion(P<0.05). There was significantly different between the two groups(P<0.05).It cloud be seen that glutamineenriched bile better than bile reinfusion.Conclusion: Glutamine enriched bile reinfusion enhance the immune function reduce the incidence of infection. This treatment can promote postoperative recovery of patients with benign obstructive jaundice.
