Study On The Model Of Chronie Disease Management In Zhangjiagang City

WHO had posted a statistic showed that,chronic diseases have become the leading causes of death worldwide, accounted for about 63% of all deaths. At the same time, WHO also predicted that,people died from chronic diseases will account for 75% of all deaths in the world by 2030, accounting for 60% of the total burden of disease. In China,situation is more serious, the incidence of chronic diseases is showing an increasing trend, there are 260 million people have been diagnosed with chronic diseases, People who died from chronic diseases accounted for 85% of all deaths, accounting for 70% of the total burden of disease. Our mankind health is threatened by chronic diseases.How to control chronic diseases and alleviate the contradiction of medical resources,In fact, countries have been exploring since 1950 s.Decades of exploration experience shows that,chronic diseases can be effectively prevented and controlled, countries are exploring the most cost-effective chronic disease management model, a variety of innovative management models continue to emerge,The typical ones include the chronic disease self-management model,the chronic disease community management model and the chronic disease information monitoring management model.In recent years, the chronic disease community management model has get more and more attention, because it was found that, carrying out health promotion and education for residents’ living behaviors is the most economical and effective way. and at this point, the community has an unparalleled advantage than hospital.Zhangjiagang City is located in the Yangtze River Delta area,economically developed.In recent years, the number of deaths caused by chronic diseases accounted for more than 80% of the total number of deaths in Zhangjiagang residents, the incidence of cancer, stroke, coronary heart disease, hypertension, diabetes and other chronic diseases increased year by year, accompanied by young tendency. To actively respond to the challenges of chronic diseases,the government of Zhangjiagang city began to explore health management of chronic disease since 1997.In 2011, grasping the Opportunity of create "National chronic disease comprehensive prevention and control demonstration area",the government of Zhangjiagang city increased investment to actively explore effective model of chronic disease management. After nearly five years of exploration, Zhangjiagang city has accumulated a wealth of experience, and initially built the information-based Trinitarian management model of chronic diseases.This thesis takes Zhangjiagang chronic disease management model as research object,analyse current situation of Zhangjiagang chronic disease management, summary experience and advanced features, propose solutions and ideas to current problems,and put forward feasible suggestions for improving the existing management model of chronic diseases.
