Objective:1、To observe the clinical curative effect of the Phlegm and Blood Stasis Qi-stagnation Syndrome Patients of Coronary Heart Disease with Carotid Artery Plaque; 2、To observe the XinTongfang intervention on serum levels of ICAM-1, VCAM-1, to explore the therapeutic mechanism of XinTongfang; 3、To investigate the intercellular adhesion molecule-1 (ICAM-1, CD54), vascular cell adhesion molecule (VCAM-1, CD 106), carotid artery plaque index (Crouse integral) relationship among the integral and coronary artery Gensini and ICAM-1, VCAM-1 and carotid artery Crouse integral.Method:60 patients were randomly divided into the XinTongfang group and the control group.The XinTongfang group was given XinTongfang,the control group was given aspirin and atorvastatin calcium.Then record the frequency of angina pectoris、the dosage of nitroglycerin、the the effective rate of ECG、the integration of TCM syndrome, and to test the expression of ICAM-1、VCAM-1 by ELLIS A.Result:1.Comparison of curative effect of angina pectoris in two groups, XinTongfang group is better than that of control group (P<0.05); nitroglycerin stop minus rate was no difference between the two groups (P>0.05); ECG efficacy was compared between the two groups had no difference (P>0.05); XinTongfang group and control group were significantly reduced the scores of TCM syndromes (P<0.01), and the XinTongfang group better than the control group (P<0.05); therapeutic index comparison, XinTongfang group is better than that of control group (P< 0.01); comparison of curative effect of TCM syndromes, XinTongfang group is better than that of control group (P<0.05).2.The XinTongfang group and the control group could decreased on C-IMT, Crouse score effect (P<0.05), but no significant differences between the two groups (P>0.05).3.XinTongfang group and control drugs both could significantly inhibit the expression of ICAM-1, VCAM-1 (P<0.01),but there was no significant difference between the two groups (P> 0.05).4.There was a linear relationship between carotid artery Crouse integral, ICAM-1, VCAM-1 and coronary artery Gensini integral; linear relationship between ICAM-1, VCAM-1 and carotid artery Crouse integral also exists.Conclusion:1.Carotid artery plaques can reflect the severity of coronary artery disease, have certain predictive value for cardiovascular events, ICAM-1, VCAM-1 can react with coronary heart disease and carotid artery plaque in patients with inflammatory reaction degree.2.XinTongfang can effectively improve the coronary heart disease patients with carotid artery plaque accumulation of phlegm and blood stasis in patients with qi stagnation syndrome clinical symptom and ECG ischemic changes.3.XinTongfang and the control drugs both could reduce the plaque of carotid artery C-IMT, Grouse integral, the two groups with similar effect.4. XinTongfang has obvious inhibition effect of expression of ICAM-1, VCAM-1, this show that XinTongfang could be reacted with each other through the inhibition of adhesion molecules, reduce arteriosclerosis inflammatory reaction process, in order to get a stable plaque and protect myocardial function, which may be one of the mechanisms of XinTongfang in treatment of coronary heart disease with carotid artery plaque.
