Objective:Dengue fever is considered one of the most serious insect-borne infectious diseases,recently,there has been no effective and targeted durgs or vaccines to prophylaxis and treatment options. This study set out to two aspects by clinical experiment observation and cell-animal experiment,hoping for that,to summarse the symptoms characteristics of dengue fever; to evaluate the effect of traditional Chinese medicine treatment of dengue fever;To explore the mechanism of traditional Chinese medicine treatment of dengue fever,Moreover, providing a basis and direction for the development of dengue fever drugs research.Methods:The clinical part:142 cases in-patients with dengue fever have been choosed based on the guideline of dengue fever,we performed a prospective cohort study of these choosed patients.Control group inplements conventional treatment and nursing care,while observation group was given corresponding treatment such as XiYanPing injection、 XueBiJing injection Shengmai Injection basing on each symptoms and TCM syndrome type in the conventional treatment foudation.In the research.each patient’s syndrome has been dariy recorded,the syndrome rates and clinical data are collected in the 0/3th/6th/9th day of observation period.By statistical analysis of the data of The length of defervescence time、the synpotom grading of TCM、outine blood count (WBC、PLT)、 liver fuction(ALT、AST)、the counts of CD3/CD4/CD8 cells in serum、cytokines in serum、 the quantity of dengue fever virus、ect.The experiment part:1) Experiment in vitro:HepG2 cells incubated by XiYanPing injection Ribavirin injection and saline injection in the way of incativate、treatment and preventive treatment.In the time of incubated of 2nd/8th/24th hour,to detect the number of the virus antigen positive cells and the level of DV2 RNA in each cell holes by IFA and q-PCR technology,to compare the antiviral effect to DV2 of each injection.2) Experiment in vivo:After SCID-HepG2 Mouse model being established, we injected XiYanPing injection Ribavirin injection and saline injection in the way of incativate、treatment and preventive treatment into the peritoneal cavity of mice.observing the normal situation(4 days) and death. To detect the virus tite、the blood routine (WBC、RBC、PLT)、the evels of thecytokines (IL-6、IL-17、IL-23、TNF-α、IFN γ)、the evel of active oxide (MDA、SOD、GSH/GSSG)Result:Result of clinical part:1.In the entire illness, it’s shoter which the defeverscence time of the observe group than that of the control group (P<0.05);the complication of the observe group is significantly reduced than the control group (P<0.05). At the 3st observing day, the blood platelet count of observe group is higer than the control group (P<0.05);At the 9th observing day, the albumin of the observe group is higher than control group (P<0.05).2.In each visit time, there shows no significant difference between the two group of the synpotom grading of TCM、grading of APACHEⅡ、white count、 aminotransferase(ALT)、aspartate aminotransferase (AST)、glutamyltranspeptidase (GGT)、creatine kinase (CK)、lactate dehydrogenase (LDH)、serum d-dimer Peripheral T lymphocyte number (CD3+、CD4+、CD8+、CD4/CD8)、number of DEVN (P<0.05)3. In each visit time,there exist no significant difference between the two group of Serum levels of cytokines、amount of peripheral blood dengue virus (P<0.05)Result of cell and animal part:In the vivtro expriment:(1)In the 24-hour-condition under XiYanPing injection,the DV2 positive cells number has been observed significantly reduced,the effect of which is similar to ribavirin, and shows a dose-dependent characteristics.in addition, it is more obvious by the way of prevent and treatment routine.(2) The XiYanPing injection is observed preventing DV2 penetrateing into HepG2 cell,which prompting that XiYanPing injection shows antiviral effect at the early infection, and works better than Ribavirin injection.(3) XiYanPing injection is observed a effect of reduce the number of DV2 into and out the cell,which sugest that XiYanPing injection may inhibited viral replication,and and works better than Ribavirin injection.In the vivo expriment:(1)Rates shows that by giving XiYanPing injection、Ribavirin injection and saline injection base on the group of treatment、prevent and treatment routine. ①It shows a great decrase that the number of DV2 in the serurm、liver、spleen of mouse treated by XiYanPing injection,Which promopt more apparent in viral replication inhibition. ② It shows no significant difference of levels of cytokines (IL-6, IL-17, IL-23, TNF α, IFN γ), except that the level of IL-6 presents a little lower than normal saline group. ③ It shows higer lelevl of the count of leucocyte and platelet, while the red cell and hemoglobin showed a downtrend but not obvious.(2)By doing HE staining with the main organ (liver、spleen、brain、small intestine) of mouse, we observed reduce in liver congestion and bleeding,while more than no diffence.Conclusion:The clinical curative effect of combining traditional Chinese and western medicine treatment of dengue fever,It is superior of clinical curative effect of the traditional Chinese medicine treatment of dengue fever compared to alone western medicine treatment. Main effect is embodied in:shorten the defervescence time, relieve symptoms, reduce the complications and improve platelet count, albumin. Traditional Chinese medicine has a effect of against dengue virus, its may inhibit viral replication and inflammatory reaction, which help protect the body. The mechanism may be related to direct inhibition of DF virus replication and ease the body’s inflammatory response.
