Part Ⅰ:study on the molecular classification of breast cancer patients and the significance of clinical pathological characteristics in Guangxi areaObjective:To analyze and compare the molecular classification and clinical pathologic characteristics of breast cancer in postmenopausal women and young women, and to provide evidence for individual treatment and prognosis.Methods:Select 377 cases of postmenopausal women with breast cancer and 102 cases of young female breast (less than or equal to 35 year old) cancer patients with IHC method was used to measure the estrogen receptor (ER), the progesterone receptor (PR), the human epidermal growth factor receptor 2 (HER2) and Ki-67 expression and molecular typing, combined with the clinical and pathological features were analyzed and compared.Results:The molecular subtypes of the two groups were mainly Luminal B, but the difference was not statistically significant (P> 0.05). Two groups of patients in the distribution of pathological type, histological grade, tumor size, lymph node metastasis, clinical stage differences were statistically significant (both P< 0.05). The expression of ER, PR, HER-2 and Ki-67 in the two groups were statistically significant (both P< 0.05).Conclusion:Postmenopausal women and young women have unique molecular subtypes and clinical pathological features, clinical treatment should be based on the prognosis of risk assessment and development of individualized treatment program.Part II:The significance of different molecular subtypes of breast cancer and peripheral blood lymphocyte subsets with clinical pathological featuresObjective By detecting the changes of peripheral blood lymphocyte subsets in patients with different subtypes of breast cancer, and explore the relationship between different subtypes of breast cancer and their clinical pathological characteristics.Methods Select affiliated Tumor Hospital of Guangxi Medical University from January 2013-2013 December of a total of 206 patients with women breast disease patients, randomly divided into for 166 cases of patients with breast cancer group (observation group, n= 166) and 40 cases of healthy people as control group (control group, n= 40),, Immunohistochemistry envision two-step was used to detect the expression of estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER2) and Ki-67, using flowcytometry to detect the two group the of peripheral blood lymphocyte subgroup of CD3+、CD4+、CD8+、CD16+CD56+、CD19+ activity, combined with the clinical and pathological features were analyzed and compared.Results compared with the normal healthy group, the percentage of CD3+and CD4+ /CD8+ was significantly decreased in the groups of Luminal A、 Luminal B type and HER-2 Over expression.(p< 0.05); In the four kinds of molecular subtypes, the activity of CD4+ and CD 19+ was significantly decreased compared with the normal healthy group (p< 0.05); The activity of NK (CD 16+CD56+) of triple negative was significantly decreased compared with the normal healthy group (p< 0.05); Histological grade Ⅱ has a significantly lower activity of CD4 +than the group of histology grade Ⅰ (p< 0.05). The histological grade Ⅲ has a significantly lower activity of NK (CD16 + CD56+) than the histological grade Ⅱ (p< 0.05). The activity of CD4+ and CD4+ /CD8+ ratio in patients with lymph node metastasis were significantly lower than the patients without lymph node metastasis (p< 0.05).Conclusion The determination of peripheral blood lymphocyte subsets in patients with different molecular subtypes of breast cancer patients have a certain significance in the state of immune function, which can contribute to the observation of the disease, curative effect monitoring and prognosis. Part Ⅲ:Expression of OPN and P21 in breast cancer and significance of molecular typing and prognosisObjective:To investigate the expression of osteopontin (OPN) and p21 in breast carcinoma and benign breast disease, analyze their significance in diagnosis and prognosis.Methods:selected from Guangxi Medical University Affiliated Tumor Hospital in 2011 January to December from a total of 53 cases of female breast disease patients were randomly divided into 43 cases of breast cancer group (observation group) and 10 cases of benign diseases group (control group, n= 10), the observation group use IHC Envision two-step detection of estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER2) and Ki-67 protein expression and molecular typing, after the two groups of paraffin tissue chip using IHC Envision two-step detection of OPN and p21 expression, combined with clinical and pathological features of data analysis.Results:The positive expression of OPN and P21 in invasive breast cancer rates were significantly higher than those of benign breast disease group (P< 0.05); The expression of OPN on Histological grading, lymph node metastasis, clinical staging, the expression of ER and PR, and recurrence and metastasis were related (P< 0.05); The expression of P21 on Histological grading, lymph node metastasis, clinical stage, the expression of HER-2 was correlated (P< 0.05); the expression of OPN on the HER-2 over expression and Basal-like was higher than that of Luminal A and Luminal B, there was significant difference between the groups (P< 0.05); OPN protein in invasive breast cancer not Luminal expression the rate is higher than that of subtype Luminal subtype, the difference was statistically significant (P< 0.05); P21 protein expression in invasive breast cancer subtype of not Luminal was lower than Luminal subtype, the difference was statistically significant (P< 0.05); due to COX regression analysis showed that the recurrence and metastasis and OPN expression were independent prognostic factors in breast cancer patients (P< 0.05). Conclusion:both of expression of OPN and p21 in breast cancer are related to the progression of breast cancer. Combined detection of OPN and p21 expression in the molecular subtypes of breast cancer have help to judge the prognosis of breast cancer, whether as a molecular sub type new marker needs further study.
