Objective:Studies have shown that the occurrence and development of tumors are closely related to the immune response of the body.The detection of peripheral blood lymphocyte subsets is an important indicator of the body's cellular immune function.The purpose of this study was to analyze the correlation between peripheral blood lymphocyte subsets and their clinicopathological characteristics in breast cancer patients,and to investigate the predictive value of preoperative detection of peripheral blood lymphocyte subsets on the severity and prognosis of breast cancer patientsMethods:Breast cancer patients admitted to our hospital for modified radical surgery from December 2018 to October 2019 were selected,and 75 patients were screened according to the inclusion criteria.In application of flow cytometry(FCM)technology to detect the preoperative patients peripheral blood lymphocyte subsets(CD3+T cells and CD4+T cells and CD8+T cells and CD4+/CD8+T T,NK cells,CD 19+B cells).A reasonable statistical method was used to analyze the correlation with the indicators reflecting the severity and prognosis of the patients,such as clinical stage,immunohistochemical index,tumor size,presence or absence of lymph node metastasis,histological grade and with or without vascular cancer embolus of the patientResults:? Immunohistochemical index:the CD3+T level of the ER positive group was significantly higher than that of the negative group(p=0.032).The level of CD19+B in the ER positive group was significantly lower than that in the negative group(p<0.001).The level of CD4+T/CD8+T in the PR positive group was significantly higher than that in the negative group(p=0.045).The level of CD19+B in the PR positive group was significantly lower than that in the negative group(p=0.022),and the differences were statistically significant.The NK level of patients in the her-2 positive group was significantly lower than that in the negative group,with a statistically significant difference(p<0.001).The CD8+T level of patients in the high-expression Ki67 group was significantly lower than that in the non-high-expression group,and the difference was statistically significant(p=0.005).?In the patients with lymph node metastasis group,CD4+T and CD3+T levels were significantly lower than those in the non-metastasis group,with statistically significant differences(p<0.05).?The levels of CD4+T and CD3+T were significantly lower in the group with vascular cancer embolus than in the group without vascular cancer embolus(p<0.05).The level of CD19+B was significantly higher than that of the control group(P=0.002).?In different tumor size groups,there were statistically significant differences in CD8+T and CD3+T.?In different histological grade groups,there were statistically significant differences between groups with CD4+T,CD3+T and CD19+B levels.?In different clinical staging groups,CD4+T and CD3+T levels were statistically different between the groups.? CD19+B can effectively predict the occurrence of lymph node metastasis in patients,the threshold value is 14.00,the AUC is 0.631(0.505?0.758),the sensitivity is 48.9%,the specificity is 86.7%.?CD19+B can effectively predict the occurrence of vascular cancer embolus in patients,the threshold value is 14.00,the AUC is 0.711(0.592?0.830),the sensitivity is 53.7%,the specificity is 88.2%Conclusion:Patients with breast cancer have immune dysfunction.In the peripheral blood of patients,there are lower levels of CD3+T cells,CD4+T cells,CD8+T cells,CD4+T/CD8+T cells and NK cells,and higher levels of CD 19+B cells,suggesting a greater degree of immune escape of breast cancer cells,resulting in more serious illness and worse prognosis.Moreover,peripheral blood CD 19+B lymphocyte level have certain predictive value for presence or absence lymph node metastasis and vascular cancer embolus in breast cancer patients.So preoperative patients with suspected breast cancer detection of peripheral blood lymphocyte subsets,can comprehensive assessment on patients with immune status,not only to understand the patient's illness severity,even for patients with lymph node metastasis and vascular tumor emboli are predicted,and whether you need for preoperative drug intervention and surgical timing of all has very important significance.At the same time,with the development of biological immunology and oncology,cellular immunotherapy has become the focus of treatment for breast cancer,especially for breast cancer patients with poor traditional treatment effect.The detection of peripheral blood lymphocyte subsets of breast cancer patients reflects the immune function status of breast cancer patients,which provides valuable Suggestions for the clinical application of immunotherapy,combined with surgery,endocrine therapy,chemoradiotherapy and molecular targeted therapy ×...
