Objective:Comparing the difference of bile M2-PK(pyruvate kinase isoenzyme type M2)expression in patients with bile duct carcinoma and benign biliary tract disease groups and researching the expression of M2-PK in bile duct carcinoma tissue to investigate its clinical application value in the diagnosis of cholangiocarcinoma.Methods:We collected bile samples from 33 patients with bile duct carcinoma and 33 patients with choledocholithiasis and choledochoodditis from July 2014 to June 2015 in digestive system department of Changzhou city first people’s hospital, detected bile M2-PK concentration with ELISA, analyzed the sensitivity and specificity of bile M2-PK for differential diagnosis of cholangiocarcinoma and benign biliary diseases and its relationship with clinical parameters.We collectioned tissue samples from 55 patients with bile duct carcinoma, including 27 cases from the extrahepatic bile duct carcinoma tissue chip, the rest of the 28 cases from patients in department of hepatobiliary surgery in the third affiliated hospital of suzhou university(changzhou first people’s hospital) between January 2010 and March 2014, use immunohistochemical method to detect the content of the M2-PK in cancerous tissue, and analysis relationship between M2-PK expression and the clinical pathological features by ?2 test.Results:Bile M2-PK concentration in patients with bile duct carcinoma were significantly higher than those in benign biliary diseases, the difference was statistically significant(P<0.001), the sensitivity,specificity and veracity of bile M2-PK for differential diagnosis of cholangiocarcinoma and benign biliary diseases were 72.73%, 75.76% and74.24%, respectively.And the concentration of the bile M2-PK were relevant to the value of total bilirubin and direct bilirubin, there was statistically significant difference(P<0.05),but it were irrelevant to the factors of gender, age, the position of the bile duct carcinoma, the value of ALT, AST, GGT, ALP, serum CA199 and CEA, there was nostatistically significant difference(P>0.05). M2-PK expression was significantly higher in bile duct carcinoma tissue than tissue adjacent to carcinoma, bile duct carcinoma tissue M2-PK expression total positive rate was 63.64%(35/55). M2-PK expression level in patients with bile duct carcinoma tissue were irrelevant to the factors of age, sex, tumor size, TNM staging(P > 0.05), but relevant to the pathological grading(P < 0.05).Conclusions:M2-PK have certain clinical significance to diagnose bile duct carcinoma and is expected to become a new tumor marker.
