Objective:Preparation of water extract from traditional Chinese Medicine Radix Daphne genkwa herbs of topical for the wound after anal fistula operation, the control of the effect on pain, inflammatory response and wound healing after anal fistula operation and explore the roots of Daphne genkwa boiled line in the treatment of hemorrhoids and fistula mechanism and to reveal the roots of Daphne genkwa cook line for the scientific basis of hemorrhoids and anal fistula treatment.Methods:Will December 2015-2016 February in Jiangsu Province Chinese medicine hospital anorectal Hospital of 40 cases of low simple anal fistula patients as research object, randomly divided into treatment group and control group, each with 20 cases in each. Consistent with two groups of patients with preoperative preparation, operation using low anal fistulotomy. Two groups of patients with postoperative treatment and postoperative hip bath cleaning treatment were the same. But postoperative dressing change in the treatment group with 20% of the roots of Daphne genkwa water extract yarn strip (1ml solution containing 0.2 g of raw materials) to fill the wound dressing, control group with baicalin ointment gauze fill a wound dressing, dressing was changed every day twice. Records are regularly postoperative anal pain score, wound secretion changes, wound infiltration liquid imprint in the number of macrophages, granulation tissue growth points, wound healing time, the data in each group were statistically comparative study. Data were analyzed by SPSS 19.0.Results: ① wound secretion score:after 3 days of treatment group and control group secretion score respectively:1.6±0.75,2.45±0.51, after 7 days of treatment group and control group secretion score respectively:1.1 ±0.64,2.15±0.67, postoperative day 10 treatment group and the control group secretion score respectively:0.35±0.49,1.4±0.6, the treatment group than the control group, three time points between groups respectively significant difference (P< 0.01). ② wound pain:after 2 days,5 days,7 days, the 14 day treatment of pain score respectively:3.65± 0.81,4.65±0.67,2.65±0.75,0.6±0.68, control group:4.65±0.99,5.9±0.91,4.9±0.71,1.75±0.97, treatment group was better than that of the control group,4 time points between groups were compared respectively, there was significant difference (P< 0.05).③Wound fluid imprint the number of macrophages:in postoperative 3 days,7 days, recorded the wound infiltration printing liquid tablets in the number of macrophages infiltration, comparison, patients in the treatment group after 7 and 10 days respectively:7.25±2.29/HP,3.95±185/HP, control group:5.35±2.08 /HP,2.10±1.17/HP, treatment group was better than that of the control group,2 time points between groups there were statistically significant differences in (P< 0.05).④the growth of granulation is:treatment group and control group after 7 days of granulation growth points were: 2.5±0.6,2.5±0.51,10 days after the surgery respectively:1.55±0.6,1.55±0.51, the 14th day after operation respectively:0.8±0.41,0.8±0.7, the 7th、10th and 14th day after operation between the two groups compared no significant difference (P> 0.05). ⑤wound healing time, wound treatment group the average healing time (days):19.35±4.16 and control group, the average healing time (days):22.1±3.9, the two groups through the data analysis of P> 0.05, no statistical difference.⑥The total efficiency after January statistics of two groups of patients with the total effective rate was 100%. In the course of treatment, no adverse events occurred.Conclusion:Roots of Daphne genkwa water extract can effectively reduce postoperative anal fistula wound exudate, postoperative wound pain relief, and in wound healing early metaphase increased the number of wound macrophages, but accelerated wound repair is not significant.
