Purpose:Through the measurement of preterm and full term pregnant women serum intercellular adhesion molecule 1(ICAM-1), tumor necrosis factor alpha(TNF alpha)expression levels, and before delivery from vaginal secretions, mycoplasma, chlamydia,Neisseria gonorrhoeae, fungi and bacteria detection, to investigate the pregnant women during early production in serum ICAM-1 and TNF alpha expression level and vaginal pathogen and premature birth, infection of preterm labor relationship, and to explore the pathogenesis of preterm birth and for the prevention and treatment of premature provide certain theoretical basis.Method : Selected January 2014- December 2015 in obstetrical department of our hospital inpatient diagnosis of preterm birth in pregnant women with 60 cases as experimental group, select the hospitalized delivery of a full-term pregnant women 60 cases as control group, experimental group were in labor before venous blood 5ml, control group in not in labor, draw venous blood 5ml, centrifuged and the serum, measuring the level of serum sICAM-1 and TNF alpha, using enzyme linked immunosorbent double antibody sandwich assay(ELISA) to detect and before delivery from vaginal secretions, pathogen detection,postpartum from fetal membrane rupture of 2cm fetch 2cm * 2cm placental disease inspection.Relatively premature delivery group and full-term group, the levels of serum ICAM-1 and TNF-a expression level differences and preterm group and full-term vaginal secretions pathogen results differences, premature group vaginal secretion and chorionic chorioamnionitis and preterm group and term group postpartum hemorrhage, puerperal infection and neonatal asphyxia of the differences.Result :(1) Premature expression of ICAM-1 and TNF-a in serum group were significantly higher than those in the control group, the difference is statistically significant(P< 0.05), premature with chorioamnionitis group, the serum levels of ICAM-1 and TNF-a expression levels were significantly higher than those of the preterm chorioamnionitis group were was statistically significant(P < 0.05),(2) Preterm placenta chorioamnionitis ratio was significantly higher than that of the full-term group.(3) In preterm labor group vaginal abnormal incidence was significantly higher than the control group.(4) Results in normal,phase I, phase II, phase III group, the level of ICAM-1 in general mean not all equal, the difference is statistically significant(P = 0.000) F=120.44; four groups are pairwise comparison, stage I group and II group, during the phase I and III were unequal number, P all< 0.05, the difference has statistical significance group pathological premature birth, four groups of patients with TNF alpha levels overall mean not all equal, F=51.19, P = 0.000, butin stage II and stage III group serum TNF- levels overall is equal to the number, the difference is not statistically significant(P = 0.34, P > 0. 05).(5) In premature group abnormal vaginal secretion group and normal group sICAM-1 values to the overall variance ranged,F=42.71, P < 0.01, the difference is statistically significant, ranging from two groups of TNF alpha value to the overall variance, F=15.92, P < 0.01, the difference is statistically significant.(6) The relationship between abnormal vaginal secretion and the amniotic membrane, =1.28,P < 0.05, the difference was statistically significant.(7) Comparison of maternal and infant pregnancy outcomes in preterm and term group, no statistical significance.Conclusion:Premature pregnant women serum sICAM-1 and TNF alpha expression levels did not increase to the pregnancy continues, biochemical indicators of labor as a predictor of infection; serum level of sICAM-1 with chorioamnionitis aggravating infection increased, serum TNF alpha levels and chorioamnionitis. Vaginal pathogen infection, increase the risk of preterm delivery, and have close correlation with chorioamnionitis; pregnancy combined detection of the serum levels of sICAM-1 and TNF alpha expression level at the same time, vaginal pathogen detection, can be used in the prediction of preterm birth.
