Objective To systematically evaluate the effectiveness and safety of acupuncture compared with drugs for migraine prevention, and facilitate the clinical application of acupuncture. Methods Searched database of Pubmed, Cochrane library, EMBASE, CBM, VIP, Wan Fang Data and CNKI from January 1989 to December 2015, screened and retrieved the randomized controlled trials(RCT) of acupuncture comparing drugs for migraine prophylaxis. And then, the included studies would be assessed and extracted data and made meta-analysis.Results A total of 7 RCTs and 1285 migraineurs were included in this study. There was a pronounced difference[RR=1.41,95%CI(1.06,1.88),P=0.02] and significant heterogeneity(P=0.07, I2=57%) in response rate during 3-4 months after randomization, during 5-6 months after randomization, it had no heterogeneity(P=0.52, I2=0%) and no significance [RR=1.18, CI(0.97,1.43), P=0.11]. For migraine days, there were significant difference between the two groups during 3-4 months [SMD=-0.30, 95%CI(-0.45,-0.16), P<0.0001] and 5-6 months after randomization [MD=-0.66, 95%CI(-1.18,-0.13), P=0.01], respectively, it showed good homogeneity(P=0.38, I2=3%),(P=0.60, I2=0%). Moreover, it was statistically s ignificant difference between acupuncture versus drug treatment in migraine frequency during 3-4 months after randomization, and intens ity of headache during 5-6 months after randomization, adverse events in research period. There were no statistical significance in intens ity of headache during 3-4 months after randomization, days with rescue medication, and migraine frequency during 5-6 months after randomization. It was difficult to synthesize for follow-up rate and partial outcomes had indelible heterogeneity. Conclusion Acupuncture is more safety than drug therapy, and it appeared to have a better clinically effectiveness on migraine prophylaxis compared with drug therapy. More high quality researches should been carried on to enhance the reliability.
