The Efficient Analysis Of ERCP In The Treatment Of Hepatic Alveolar Echinococcosis With Obstructive Jaundice

Objective:To determine the clinical efficacy of Endoscopic Retrograde Choledochopancreatography(ERCP) in the treatment of hepatic alveolar echinococcosis with obstructive jaundice,and to analyze the influencing factors for ERCP Postoperative Complications,so we can take effective measures early.Methods:we Collected 16 patients of hepatic alveolar echinococcosis with obstructive jaundice confirmed by imaging information during the period from January 2014 to November 2015 in hepatopancreat obliliary of Affiliated Hospital of Qinghai University.All of them were treated therapeutic ERCP,record the intraoperative imaging characteristics,serum levels of total bilirubin,alanine transaminase,aspartate aminotransferase,alkaline phosphatase and γ-glutamyl transferase were determined before and 1 week,1 month after ERCP,the curative effects and complications were also observed after ERCP.Do single factorχ2-test analysis find out the influencing factors of PEP and postoperative biliary infection.Results:Through the electronic medical records were collected 16 patients,all patients were successfully treated.ERCP showed biliary tract invasion.Jaundice symptoms disappeared gradually after ERCP in 16 patients. Compared with preoperation,Serum levels of total bilirubin,alanine transaminase,aspartate aminotransferase,alkaline phosphatase and γ-glutamyl transferase apparently decreased in all the patients 1 week,1 month after ERCP.16 cases of ERCP postoperative complications occurred in 6cases,1 cases of high amylase hematic disease,2cases of PEP,postoperative biliary infection in 3 cases, no hemorrhage and perforation occurred because of ERCP.In the univariate analysis,a history of acute pancreatitis and guide wire entry repeatedly into the pancreatic duct was relevant factor of PEP(P<0.05).A history of biliary tract infection(P<0.05)and Putting the trestle into the bile duct(P<0.05) were relevant factor of postoperative biliary infection.Conclusion:ERCP can effectively relieve jaundice,improve liver function.So it Can be used as One of the effective treatment method in hepatic alveolar echinococcosis with obstructive jaundice. At the same time,we should take effective measures to prevent and control the influence factors of PEP and postoperative biliary infection.
