Objective:To observe the clinical curative effect of open reduction and internal fixation and conservative treatment for traumatic multiple fractured ribs, and evaluate the clinical curative effect of operative opportunity.Methods:The clinical data of 120 cases suffered from traumatic multiple fractured ribs were retrospectively analyzed,who were selected from the Second hospital of Shanxi Medical University from June 2011 to June 2015.Among them,57 cases were treated by conservative treatment, who were servered as Group of non-surgery. The other 63 cases were treated by open reduction and internal fixation surgery,who were servered as Group of surgery. Incidence of lung infection and atelectasis after injury,analgesic drug use time and conditions of ribs fracture healing in two groups were compared.The surgical treatment group was divided into Group A(Surgical treatment group within 6 days,32cases)and Group B(Surgical treatment group after 6 days,31 cases). Incidence of lung infection and atelectasis after injury,incidence of postoperative incision infection with full-thickness split,analgesic drug use time,thoracic cavity closed drainage time and conditions of ribs fracture healing in two groups were compared.Results:Incidence of lung infection and atelectasis after the injury in Group of surgery wassignificantly lower than that in Group of non-surgery,indicating statistically significant difference(x2=4.126,P<0.05;5.632,P<0.05).Analgesic drug use time in Group of surgery was significantly less than that in Group of non-surgery,indicating statistically significant difference(Z = 6.041,P<0.05).Conditions of ribs fracture healing in Group of surgery was obviously better than that in Group of non-surgery,indicating statistically significant difference(x2=22.982,P<0.05;x2=16.714,P<0.05;x2=4.574,P<0.05).Incidence of lung infection and atelectasis after injury in Group A was lower than that in Group B,indicating statistically significant difference(x2=4.510,P<0.05;3.946,P<0.05).Analgesic drug use time and thoracic cavity closed drainage time in Group A were significantly less than those in Group B, indicating statistically significant difference(Z=-5.344,P<0.05;-4.088,P<0.05).Incidence of postoperative incision infection with full-thickness in Group A was slightly lower than that in Group B,but there was no statistically significant difference(x2=0.426, P > 0.05).There was no significant difference about conditions of ribs fracture healing between Group A and Group B,indicating statistically non-significant difference(x2=0.003,P>0.05;x2=0.003,P>0.05).Conclusion:The surgical treatment of open reduction and internal fixation for traumatic rib fractures had better clinical curative effects than conservative treatment, and early surgical treatment can furthermore improved symptoms,reduce the incidence of complications after injury.
