To Compare The Clinical Effect Of Surgical Treatment With Non-operative Treatment Of Traumatic Flail Chest

Objective To compare the clinical effect of Ti-Ni shape memory alloy embracing fixator T-shaped plate in the surgical treatment of traumatic flail chest with non-operative treatment of traumatic flail chest.Methods A review was made on 412 patients with traumatic flail chest treated from August 2014 to December 2015 in Our hospital, and 294 eligible cases were enrolled in the study(AIS?3). There were 134 patients in surgical treatment group and 160 patients in non-operation group. Clinical results and incidence of complications were compared between the two groups.To compare the efficacy and complications between the surgical treatment group and the non-operation treatment group in the same period.Results In operation group, except 2 cases died merger late-onset brainstem hemorrhage, other cases were cured. The complications of chest wall deformity, length of hospital stay, pain sensation, dosage of analgesics, breathing machine auxiliary time, pulmonary infection, atelectasis less than those cases in non-operative group(P<0.05). C-reactive protein levels at the 7th day after treatment significantly was superior to non-surgical group(P <0.05).There was no significant difference in gender, age, multiple injury severity score(ISS), number of rib fractures, and concise chest trauma score(AIS)(P>0.05).Conclusion The clinical outcome of Ti-Ni shape memory alloy embracing fixator in the surgical treatment of traumatic flail chest is better than non-operative treatment of traumatic flail chest.The operation group has the advantages of shorter length ofhospitalization, short time of use of breathing machine, low pulmonary infection rate, low incidence of ARDS, low incidence of thoracic deformity, and has high clinical application value.
