Objective: To explore the clinicopathological significance in papillary thyroid carcinoma through examining and comparing expressed ki-67 and BRAFV600E mutation in cancerous and adjacent normal thyroid tissues, as well as the impact of expressed ki-67 and BRAFV600E mutation on the diagnosis, treatment and prognosis of papillary thyroid carcinoma.Methods:(1) We chose 73 cases of papillary thyroid carcinoma and adjacent normal thyroid tissues 10cases; all of samples were the formalin-fixed and paraffin embedding tissues.(2) Immunohistochemistry(Envision method) was used to detect the expression of ki-67; the mutant allele specific amplification method(MASA-PCR) was used to detect the BRAFV600E point mutation;(3) We collected 73 cases of clinical data and analysed their clinical pathological characteristics;(4) Statistical analysis was performed using SPSS(version 17.0) statistical software package. Qualitative data were analyzed using chi-square test,Spearman rank correlation test and Kappa consistency detection was used to analyze the correlation between ki-67 expression and BRAFV600E mutation in thyroid carcinoma and its clinical pathological significance.Results:(1) There was no expression of ki-67 and BRAFV600E mutation in adjacent normal thyroid tissue;In papillary thyroid carcinoma, ki-67 low expression and BRAFV600E mutation rate was high, we considered that BRAFV600Emutation and the expression of ki-67 protein could be as a index of diagnosis and treatment of papillary thyroid carcinoma.(2) The expressions of ki-67 in papillary thyroid carcinoma tissues become significantly higher along with the cancer focus to lager, the extracapsular invasion, lymph node metastasis,bilateral thyroid carcinoma.The BRAFV600E mutation were correlated with cancer focus, lymph node metastasi, the extracapsular invasion and clinical stage. ki-67 expression and BRAFV600E mutation prompted with papillary thyroid carcinoma development, invasion and metastasis.(3) There was relationship between ki-67 protein expression and BRAFV600E mutation in papillary thyroid carcinoma(P <0.05), both could be as clinical index of judging the prognosis of patients with papillary thyroid carcinoma.(4)Comparison group were using immunohistochemical method to detect BRAFV600E mutation, the result of the two kinds of detection methods have better degree agreement.Conclusions:(1) The BRAFV600E mutation is correlated with the clinical pathological progress of papillary thyroid carcinoma and poor prognosis;ki-67 protein expression with papillary thyroid carcinoma is closely related to the ability of invasion and metastasis;(2) There is relationship between the BRAFV600E mutation and ki-67 protein expression of four groups in papillary thyroid carcinoma tissues,prompts both detection can be used as papillary thyroid carcinoma diagnosis, treatment and prognosis judgement index.
