Object: To use experimental research method, a community design as experimental group and another, the scale and level of a community design as control group, compared with two patients before and after the intervention, and the intervention of two groups of patients after in diabetes knowledge and self-management ability, quality of life and blood glucose control differences. To explore the application effect of multidisciplinary team mode in disease management in community patients with diabetes, and provide a theoretical basis for the improvement of the management pattern and the level of the prevention and control of diabetes in community.Methods: The research method of class test. In accordance with the inclusion of exclusion criteria for patients with type 2 diabetes in accordance with the community groups, cultural communities for the experimental group, the number of cases for 85 cases; entrepreneurial community for the control group, the number of cases in 91 cases. The experimental group was intervened by the multi discipline team, and the control group was intervened with the current management mode of the community. Respectively in intervened before and 6 months after using the diabetes knowledge test questionnaire ", the scale of quality of life in patients with diabetes specific volume, the diabetes self management behavior form" of two groups of patients were investigated, the table changes in the score, to multidisciplinary team model to evaluate the effect of community diabetes disease management.Results:(1) intervention after six months, the two groups before and after intervention in patients with diabetes knowledge score difference had statistical significance(P < 0.05). Experimental group after intervention diabetes knowledge success rate increased, and the difference was statistically significant(P < 0.05); Control group after the intervention diabetes knowledge success rate increased, but there was no statistically significant difference(P > 0.05). Diabetes knowledge score difference after intervention, intervention group is significantly higher than the control group, the difference was statistically significant(P < 0.05).(2) the intervention after six months, the score of two groups of patients in the self-management ability and each dimension scores are improved, and the difference was statistically significant(P < 0.05). About two groups of self-management ability score difference before and after intervention, intervention group is significantly higher than the control group, there was no statistically significant difference in addition to the foot care dimension(P > 0.05), and the rest of the dimension scores and total score difference had statistical significance(P < 0.05). After six months,(3) the intervention group in the quality of life scores before and after the intervention and the dimension scores were statistically significant(P < 0.05), the control group in the quality of life scores before and after the intervention and the dimension scores were increased, but there was no statistically significant difference(P > 0.05). About two groups of patients before and after the quality of life score difference, experimental group was higher than the control group, the difference was statistically significant(P < 0.05).(4) two groups of patients before and after the intervention of fasting blood sugar and glycated hemoglobin values differences were statistically significant(P < 0.05). After intervention, on fasting blood glucose and glycosylated hemoglobin difference comparison of experimental group was higher than the control group, the difference was statistically significant(P < 0.05).Discussion:(1) based on the multidisciplinary team management mode of community diabetes disease management can improve the patient’s level of blood sugar control, keep the patients blood sugar levels steady state;(2) the multidisciplinary team mode can improve the level of patients with diabetes knowledge, self-management ability, diabetes health literacy and social capital level;(3) based on multi-disciplinary community diabetes management effect is superior to the conventional pattern of community management of diabetes, suggest that in conventional management mode at the same time promoting multidisciplinary team management mode, to realize the scientific nature, diversity and pertinence of disease management;(4) community diabetes patients based on the influence factors of the multidisciplinary team model for disease management are various, hints of community patients with diabetes should be targeted and personalized disease management measures.
