Objective: The aim of this study was to investigate the role of multidisciplinary team(MDT)management in the treatment of patients with oral cancer with systemic diseases by using a database in the Department of Oral and Maxillofacial Surgery of Qingdao University Affiliated Hospital.Methods: The study was conducted between September 1st,2016 and December 31,2018.The clinical data of 60 patients who participated in oral MDT consultation,including basic data,oral cancer TMN staging,tumor location,combined systemic disease and MDT treatment plan.According to the patient's characteristics of oral cancer and systemic conditions,MDT members develop treatment plans of primary tumor and systemic diseases,and summarize the implementation methods and effects of patients' comprehensive diagnosis and treatment.Results: Sixty patients with oral cancer underwent different treatments for systemic diseases,surgery,radiotherapy,chemotherapy,and immunotherapy according to the relevant diagnosis and treatment plan developed by MDT.MDT provided personalized surgical treatment for 45(75%)patients with well-tolerable surgery,including enlarged tumor resection,neck dissection,flap transfer,etc.;34 cases(56.67%)were combined Blood pressure patients proposed drug intervention and other control in the blood pressure program,blood pressure was controlled below 140/90 mmHg after treatment with hypertension;13 cases(21.67%)with coronary heart disease patients proposed perfect coronary spiral CT angiography and other examinations,coronary In the treatment of arterial interventional surgery,1 patient underwent coronary artery bypass grafting andtumor resection,and the other 4 patients underwent tumor resection after PCI;2 patients(3.33%)with severe arrhythmia presented drug intervention,installation of temporary pacemakers and other related treatment opinions;23 cases(38.34%)of diabetic patients were provided with rationalized control of blood glucose,including 5 cases of oral metformin and other drugs before surgery,8 patients were treated with subcutaneous injection of insulin before surgery,10 The patients were treated with the "diabetes insulin pump treatment expert group" led by the endocrinology department in our hospital.Cases of 19 patients(31.67%)with respiratory disease were treated with corresponding pulmonary function tests and postoperative complications.Two patients with preoperative respiratory disease had descending pneumonia after surgery.The infection was effectively controlled after the replacement of antibiotics in the culture results.Conclusion: The role of multidisciplinary team members are important in the diagnosis and treatment of patients with oral cancer.The diagnosis and treatment plan made by the multidisciplinary consultation team can provide a personalized treatment for patients with oral cancer and systemic diseases.Oral and maxillofacial surgeons have improved their ability to recognize systemic diseases,and promote communication and cooperation among various disciplines through multidisciplinary discussion.
