Objective: Thyroid carcinoma is a common malignant tumor of head and neck,common in women,the proportion of female and male is about 3:1.Through the experiment of the detection of P53,Ki67,Survivin expression in thyroid carcinoma,and to explore its clinical relevance of thyroid cancer.Method: Collected the thyroid tissue of 2013-2015 that with complete clinical and pathological information from the pathology department of the Third Hospital of Hebei Medical University,all cases were as the first,before the operation had not received radiotherapy and chemotherapy.Selected 40 cases of thyroid carcinoma,including 33 cases of papillary carcinoma,4 cases of follicular carcinoma,3 cases of medullary carcinoma,as control group,40 cases were nodular goiter.According to the clinical data of selected cases of thyroid cancer and male female ratio is 12:28,age 19-74 years old,according to the age in 45 as a dividing line,the ratio of ?45 and >45 is 20:20,which associated with lymph node metastases were found in 17 cases.The immunohistochemistry was used to detect the expression of P53,Ki67,Survivin in thyroid carcinoma and nodular goiter.The data were analyzed by SPSS22.0,application of chi square test and Spearman correlation test.Results:1 Immunohistochemical results of P53The positive expression of P53 in thyroid carcinoma was 30 cases,the positive rate was 75%,the positive expression was 6 cases in nodular goiter,the positive rate was 15%,compared the expression of P53 in thyroid carcinoma and nodular goiter,the difference was statistically significant(P<0.05).In the thyroid carcinoma,the positive expression of P53 in male was 10 cases,the positive expression rate was 83.33%,the positive expression in female was 20 cases,the positive expression rate was 71.43%,the positive expression of P53 in age of >45 was 15 cases,the positive expression rate was 75%,the positive expression in age of ?45 was 15 cases,the positive expression rate was 75%,the positive expression of P53 in patients with lymph node metastases was 16 cases,the positive expression rate was 94.12%,the positive expression patients without lymph node metastases was 14 cases,the positive expression rate was 60.87%,the above results showed that the expression of P53 in thyroid carcinoma with gender and age had no significant difference(P>0.05),but there were significant differences in lymph node metastasis(P<0.05).2 Immunohistochemical results of Ki67The positive expression of Ki67 in thyroid carcinoma was 28 cases,the positive rate was 70%,the positive expression was 8 cases in nodular goiter,the positive rate was 20%,compared the expression of Ki67 in thyroid carcinoma and nodular goiter,the difference was statistically significant(P<0.05).In the thyroid carcinoma,the positive expression of Ki67 in male was 8 cases,the positive expression rate was 66.67%,the positive expression in female was 20 cases,the positive expression rate was 71.43%,the positive expression of Ki67 in age of >45 was 16 cases,the positive expression rate was 80%,the positive expression in age of ?45 was 12 cases,the positive expression rate was 60%,the positive expression of Ki67 in patients with lymph node metastases was 16 cases,the positive expression rate was 94.12%,the positive expression patients without lymph node metastases was 12 cases,the positive expression rate was 52.17%,the above results showed that the expression of Ki67 in thyroid carcinoma with gender and age had no significant difference(P>0.05),but there were significant differences in lymph node metastasis(P<0.05).3 Immunohistochemical results of SurvivinThe positive expression of Survivin in thyroid carcinoma was 25 cases,the positive rate was 62.5%,the positive expression was 6 cases in nodular goiter,the positive rate was 15%,compared the expression of Survivin in thyroid carcinoma and nodular goiter,the difference was statistically significant(P<0.05).In the thyroid carcinoma,the positive expression of Survivin in male was 6 cases,the positive expression rate was 50%,the positive expression in female was 19 cases,the positive expression rate was 67.86%,the positive expression of Survivin in age of >45 was 14 cases,the positive expression rate was 70%,the positive expression in age of ?45 was 11 cases,the positive expression rate was 55%,the positive expression of Survivin in patients with lymph node metastases was 14 cases,the positive expression rate was 82.35%,the positive expression patients without lymph node metastases was 11 cases,the positive expression rate was 47.83%,the above results showed that the expression of Survivin in thyroid carcinoma with gender and age had no significant difference(P>0.05),but there were significant differences in lymph node metastasis(P<0.05).4 The relationship between the expression of P53,Ki67 and Survivin in thyroid carcinomaThe expression of p53 and Ki67 was positively correlated in thyroid carcinoma(r = 0.389,P = 0.013<0.05).the expression of p53 and Survivin had no significant correlation(r=0.023,P=0.890 > 0.05),the expression of survivin and Ki67 no significant correlation(r=0.200,P = 0.215 > 0.05)Conclusions:1 P53,Ki67 and Survivin in thyroid carcinoma than in nodular goiter had higher expression,so P53,Ki67,Survivin may be related to the benign and malignant thyroid tumors,they can be used as the indicators of benign and malignant thyroid tumor differentiation.2 The expression of P53,Ki67 and Survivin in thyroid carcinoma has nothing to do with sex and age,but related with lymphatic metastasis.prove that p53,Ki67,survivin may play a role in thyroid cancer lymph node metastasis3 There was a positive correlation between expression of P53 and Ki67 in thyroid carcinoma,and the expression of Ki67 and Survivin,P53 and Survivin in thyroid carcinoma had no correlation.
