Objectives: By studying the expression of CD21,CD23,CD35 in the nasopharyngeal carcinoma tissue related to EB virus,analyze its clinical significance.Further for providing some references for the clinical pathological distinguish diagnosis between nasopharyngeal carcinoma and follicular dentritic cell sarcoma of nose pharynx ministry.Method: Collected the pathology biopsy which was diagnosed with type(nasopharyngeal)undifferentiated non cornification carcinoma among the outpatient and inpatient in the first affiliated hospital of guangxi medical university and guangxi university of Chinese medicine affiliated rui kang hospital about 87 cases and 20 cases of chronic inflammation of nasopharyngeal mucosa from 2012 to 2015,with two senior title of professional physicians reading respectively to review the original pathological diagnosis.Detect the selected cases with EBER immunohistochemical ion of in situ hybridization and two-step immunohistochemical of CD21,CD23 and CD35.Focus on analyzing the three molecules expression in EBER positive in nasopharyngeal carcinoma tissue,and discuss the EB virus associated nasopharyngeal carcinoma(NPC)’s pathological diagnosis meaning about the three kinds of molecular.And compare the results with follicular dentritic cell sarcoma in nose pharynx ministry tissues in literatures report about the expression of the three kinds of molecular.To explore the relationship about the expression of the three kinds of molecular between nasopharyngeal carcinoma and nasopharyngeal follicular dentritic cell sarcoma.Results:1、EBER ’s expression in nasopharyngeal carcinoma tissuesEBER’s positive expression rate in the nasopharyngeal carcinoma tissue among the first affiliated hospital of guangxi medical university and guangxi university of Chinese medicine affiliated rui kang hospital is 96.55%(84/87),higher than that of nasopharyngeal mucosa of chronic inflammation’s70.00%(14/20).EBER’s expression in nasopharyngeal carcinoma tissue and nasopharyngeal mucosal chronic inflammation has significant statistical difference between the two groups(P < 0.01).2、CD21’s expression in nasopharyngeal carcinoma tissue with EBER positiveCD21’s positive expression rate in the nasopharyngeal carcinoma tissue with EBER positive among the first affiliated hospital of guangxi medical university and guangxi university of Chinese medicine affiliated rui kang hospital is 3.57%(3/84),higher than that of nasopharyngeal mucosa ofchronic inflammation’s 0.00%(0/20).3、CD23’s expression in nasopharyngeal carcinoma tissue with EBER positiveCD23’s positive expression rate in the nasopharyngeal carcinoma tissue with EBER positive among the first affiliated hospital of guangxi medical university and guangxi university of Chinese medicine affiliated rui kang hospital is2.38%(2/84),higher than that of nasopharyngeal mucosa of chronic inflammation’s 0.00%(0/20).4、CD35’s expression in nasopharyngeal carcinoma tissue with EBER positiveCD35 has no expression in nasopharyngeal carcinoma tissue with EBER positive,also there is no expression in the chronic inflammation of nasopharyngeal mucosa.5、The clinical diagnostic value of CD21,CD23,CD35 molecular in the tissue of EBER express positive nasopharyngeal carcinomaThe highest sensitivity: separate test on CD21(30.00%);The highest specificity: joint detection of CD23 and CD35(97.52%);The biggest positive predictive value: joint detection of CD21 and CD35(37.50%);The biggest negative predictive value: joint detection of CD23 and CD35(96.92%);The highest diagnostic accuracy : combined detection of CD23 and CD35(94.64%).Conclusion:1、 Epstein-barr virus infection is closely related to the occurrence of nasopharyngeal carcinoma.2、 CD21 and CD23 in the EB virus associated nasopharyngeal carcinoma tissue have lower expression rate(3.57% and 2.38%),and CD21 and CD23 in the nasopharyngeal follicular dentritic cell sarcoma reported literaturely higher expression rate(93% and 63%),and epstein-barr virus associated nasopharyngeal carcinoma tissues have focal CD21 and CD23 positive expression,and the nasopharyngeal follicular dentritic cell sarcoma have diffuse positive expression,the two points above can be used as EBER,CD21,CD23 positive expression ’s nasopharyngeal carcinoma and follicular dentritic cell sarcoma in nose pharynx ministry ’s clinical pathological diagnosis reference.3、 CD35 did not see positive expression in nasopharyngeal carcinoma specimens within the scope of the study,and the expression rate in follicular dentritic cell sarcoma in nose pharynx ministry is 89%literaturely,however thus could treated as a reference in differential diagnosis among nasopharyngeal carcinoma and nasopharyngeal follicular dentritic cell sarcoma.4、 For the similar morphology of epstein-barr virus associated with nasopharyngeal carcinoma(NPC)cases and nasopharyngeal follicular dentritic cell sarcoma,the joint detection of CD21,CD23 and CD35 molecules and comprehensive analysis and combined with other related immunohistochemical results helps the clinical differential diagnosis between nasopharyngeal carcinoma and nasopharyngeal of follicular dentritic cell sarcoma.
