| Background and Objective:Chronic obstructive pulmonary disease(Chronic Obstructive Pulmonary Disease COPD)is a respiratory system disease,it is characterized by chronic lung inflammation and Systemic inflammatory.Inflammation is plays an important role in progressive、comorbidities、worsening,exacerbations of COPD,so the inflammatory markers have the potential to be the current one kind of effective and improve methods for the diagnosis of COPD.Location inflammation of the lung was resulted by inflammatory cells,with the aggressive expression of cytokines and chemokines,enzymes,growth factors and adhesion molecules.Systemic inflammation is another common nonspecific findings in chronic obstructive pulmonary disease(COPD).The patients of acute excerbation of chronic obstructive pulmonary disease that they were in infection,the patient usually have the clinical ancident events,even the patients died.The study research that C-reactive protein,pcalcitonin and fibrinogen of chronic obstructive pulmonary disease was changed and correlated,The purpose is to improve the early diagnos of AECOPD(Acute Exacerbation of Chronic Obstructive Pulmonary Disease AECOPD)for a lot of doctors,can guide the clinical treatment.Methods:Collected from January 2014 to December 2015 at the First Affiliated Hospital Medical University and inpatients diagnosis of chronic obstructive pulmonary disease,and meet criteria of 97 cases,COPD group was divided into two groups According to the acute exacerbation or not :acute exacerbation of chronicobstructive pulmonary disease group(AECOPD group,n=58),stable chronic obstructive pulmonary disease group(SCOPD group,n=39),At the same time,medical center of First Affiliated Hospital of Dalian Medical University,healthy persons were collected(control group,n=29)。Records of COPD patients,gender,age,The plasma C-reactive protein,Pcalcitonin and fibrinogen.Peripheral vein of COPD patients was draw to measured C-reactive protein,Pcalcitonin and fibrinogen on an empty stomach condition,and radial artery of COPD patients was extracted for testing the blood gas analysis,Peripheral vein of control group was draw to tset C-reactive protein,Pcalcitonin and fibrinogen on an empty stomach condition.To observe the differenence between with CRP、PCT、Fib in the AECOPD group、SCOPD group 、Control group and the CRP、PCT、Fib correlation with blood gas analysis in AECOPD group、SCOPD group.All date have been processed and analyzed by SPSS 17.0,((?)±s) to demonstrate the result.Among the groups were compared to use t test,CRP、PCT、Fib and PaO2、PaCO2 using Pearson correlation analysis.If P<0.05,there is statistical difference.If P<0.01,there is statistically significant difference.Results:1.In this study,two set of data on age,gender there was no statistical difference(P>0.05).the study of acute exacerbation of chronic obstructive pulmonary disease group was found that :CRP 59.27 ± 20.58 mg/L,PCT is 0.45 ± 0.12 ng/mL,FIB was4.90 ± 1.20 g/L,PaO2 58 ± 10 mmHg,PaCO2 54 ±14 mmHg,Aged 73 ± 9;In stable chronic obstructive pulmonary disease group CRP was 21.00 ± 15.95 mg/L,PCT0.14 ± 0.05 ng/mL,Aged 67 ± 10,FIB was 3.63 ± 0.80 g/L,PaO2 67 ± 10 mmHg,PaCO2 45 ± 10 mmHg.Control group : CRP was 4.04 ± 2.25 mg/L,PCT is 0.04 ±0.018 ng/mL,FIB was 2.56 ± 0.39 g/L,63 ± 10 years old;2.The plasma fibrinogen and C-reactive protein level of AECOPD group were higher than control group,there is statistically significant difference(P < 0.01).Pcalcitonin level of AECOPD group,compared with control group,there is statistical difference than control group(P < 0.05).3.The plasma fibrinogen and Pcalcitonin level of SCOPD group were higher thancontrol group,there is statistical difference than control group(P < 0.05).,C-reactive protein level of SCOPD group,was compared with control group,there is statistically significant difference(P < 0.01).4.The plasma fibrinogen,C-reactive protein and Pcalcitonin level of AECOPD group,was significantly higher than SCOPD group,there is statistical diffenerce(P <0.05);The PaCO2 of blood gas analysis of AECOPD group,was compared with SCOPD group,there is statistical difference(P < 0.05),the PaO2 of AECOPD group,was compared with SCOPD group,there is statistical difference(P < 0.05).5.Pearson correlation analysis showed that the plasma fibrinogen of patients with AECOPD group was positively correlated with C-reactive protein(P < 0.01,R=0.414)there is statistical difference,was negatively correlated with PaO2(P <0.01,R=-0.477),there is statistical difference.The plasma fibrinogen was no correlated with Pcalcitonin and PaCO2(P > 0.05),there is no statistical difference;The plasma C-reactive protein was negatively correlated with PaO2(P < 0.01,R=-0.407),there is statistical difference.C-reactive protein was positively correlated PCT(P <0.01,R=0.733),there is statistical difference.C reactive protein was no correlated with PaCO2(P > 0.05),there is no statistical difference,Pcalcitonin was no correlated with PaO2 and PaCO2(P > 0.05),there is no statistical difference.6.Pearson correlation analysis showed that the plasma Fib of SCOPD group was positively correlated with CRP and PCT(P<0.01,R(CRP)= 0.494,R(PCT)=0.728),there is statistical difference;Plasma Fib was negatively correlated with PaO2(P < 0.01,R=-0.418),there is no statistical difference;Fib was no correlated with PaCO2(P > 0.05),there is no statistical difference;Plasma CRP was positively correlated with PCT(P < 0.01,R=0.454),there is statistical difference;Plasma CRP was negatively correlated with PaO2(P < 0.05,R=-0.322),there is statistical difference;The plasma CRP was no correlated with PaCO2(P > 0.05),there is no statistical difference;PCT was no correlated with PaO2 and PaCO2(P > 0.05),there is no statistical difference.Conclusion:1.The expression level of plasma fibrinogen,C-reactive protein and Pcalcitoninin patient with acute exacerbation of chronic obstructive pulmonary disease was higher than in control group,The expression level of plasma fibrinogen,C-reactive protein and Pcalcitonin in patient with stable chronic obstructive pulmonary disease group was higher than in control group,The expression level of plasma fibrinogen,C-reactive protein and Pcalcitonin in patient with acute exacerbation of chronic obstructive pulmonary disease was higher than in stable chronic obstructive pulmonary disease group,It suggest that CRP and PCT and Fib have the sensitivity and stability for inflammation,can help to the early diagnosis of AECOPD.2.The patients of AECOPD group patients or SCOPD group,CRP and Fib were negative correlation with the oxygen partial pressure,they can be regarded as a risk factors for lung function. |
