Objective: To evaluate the effect of walking ability of cerebral apoplexy hemiplegia patients by the isokinetic muscle strength training.Methods: A total of 60 patients with cerebral apoplexy hemiplegia hospitalized in rehabilitation medicine department were selected to subjects and divided into two groups randomly, which were observation group(30 cases) and control group(30 cases). The inclusion criteria of this research were first-episode stroke, without severe cognitive impairment, hemiplegic side lower limb muscle strength at level 3 and above. The patients in control group were treated by routine rehabilitation training method. The patients in observation group were treated by the train of up-regulation the isokinetic trunk back muscle strength and side lower limb speed strength based on the routine rehabilitation training. The indexes of comprehensive motor function(the simplified Fugl- Meyer) score, balance function(Berg balance scale), stand up-walk timing test(timed “up – go” test, TUGT), flexion and extension muscle of waist and knee isokinetic testing peak torque back flexion and extension(peak-torque,PT) and the peak torque ratio flexor evaluation(F/E) were evaluated to all patients at pre-therapy and post-therapy 8 weeks respectively. The above indexes at pre-therapy and post-therapy 8 weeks were compared between two groups.Results: 1. The indexes of lower limbs' Fugl-Meyer, Berg balance scale, TUGT scale, PT scores of waist knee flexion and extension muscle isokinetic testing and F/E scores were no different between two groups at pre-therapy(P>0.05). 2. The increasing amplitude of lower limbs' Fugl-Meyer in observation group were higher than in control group after 8 weeks treatment(P<0.05); 3. The increasing amplitude of Berg balance scale in observation group were higher than in control group after 8 weeks treatment(P<0.05); 4. The decreasing amplitude of TUGT scale in observation group were higher than in control group after 8 weeks treatment(P<0.05); 5. The increasing amplitude of PT scores of flexion and extension muscle isokinetic testing of waist and knee in observation group were higher than in control group after 8 weeks treatment(P<0.05); 6. The increasing amplitude of F/E of flexion and extension muscle of waist were no differen between two groups after 8 weeks treatment(P>0.05); The increasing F/E of flexion and extension muscle of knee in observation group were higher than incontrol group after 8 weeks treatment(P<0.05);Conclusion: 1. The muscle force of trunk core position in obseravation group was increased by the train of the isokinetic trunk back muscle strength, and it also can increase the balance function of patients in obseravation group. 2. The muscle force of flexion and extension muscle of knee were increased by the train of lower limbs the isokinetic muscle strength training, and the stability of side knee joint and the motor function of lower limbs were also increased. The TUGT was shortened. 3. The walking ability can be improved by the train of up-regulation the isokinetic trunk back muscle strength and side lower limb speed strength based on the routine rehabilitation training, and the phenomenon of hypermyotonia was not obvious.
