Observation Of The Efficacy Of Intra-articular Injection Of Sodium Hyaluronate Under The X-ray Guided Combined With Internal Heat Needle In The Treatment Of Osteoarthritis Of Hip Joint

PurposeOsteoarthritis is a common chronic bone and joint disease,is a degeneration of articular cartilage and secondary bone hyperplasia is characterized by recurrent pain,joint swelling,joint deformity,progressive,movement disorder as the main clinical manifestations of the disease.With the accelerated aging of the population,the number of degenerative osteoarthritis patients is increasing.As a large weight-bearing joint in the human body,the prevalence of hip joint is only next to that of the knee joint.The anatomy of the hip is characterized by its deep position,tight joint capsule and abundant blood vessels and nerves,the lack of effective protection means in early stage of disease development,to the development of advanced disease seriously affect the quality of life and must be artificial joint replacement surgery,resulting in additional economic burden to the family.Therefore,non surgical interventions are effective and feasible in the early stage of hip OA to alleviate clinical symptoms,improve joint movement disorder and slow joint structure degeneration,prevent or delay joint replacement surgery,has positive clinical significance.To symptomatic hip OA patients,we offen use acupuncture and massagetherapy,intraarticular injection of sodium hyaluronate,oral glucosamine capsules and rehabilitation exercise and conservative treatment in clinical,however,due to the single therapy clinical effect is limited,so a variety of clinical methods are often integrated.This study selected randomized control study,according to the OA hip patients with hip joint cavity puncture and injection of sodium hyaluronate in the treatment for the control group,the clinical curative effect analysis of imaging guided hip joint cavity puncture injection of sodium hyaluronate combined with heat for hip OA,verify the heat needle combined with the clinical feasibility,safety and superiority of intra-articular sodium hyaluronate injection in the treatment of hip OA,for the early and middle hip OA conservative treatment provides a kind of thinking,and hope it can be widely applied in clinical practice.MethodA total of 85 patients selected from the rehabilitation center of Shiyan Taihe Hospital from May 2015 to February 2017 diagnosed as osteoarthritis of the hip were randomly divided into control group 42 cases,given the sodium hyaluronate injection needle therapy.In the observation group,43 cases were treated with intra injection of sodium hyaluronate combined with internal heated Awn Needle therapy.The clinical efficacy,VAS pain score,Harris hip score and WOMAC score were observed before and after treatment in the 2 groups.The observation indexes of two groups were statistically analyzed.ResultsTwo groups of patients after treatment,the observation data were analyzed by SPSS21.0 statistical software.Results: in group 4W,after12 W,the total efficiency of the control group were 50% and 72.5%,the experimental group were 72.5% and 90%,two groups of patients withthe total effective rate was increased,the comparison of long-term efficacy in the group were better in the near future curative effect.The differences were statistically significant(P<0.01);and compared with the time between the two groups in total effective rate of the experimental group were significantly higher than the control group,the differences were statistically significant(P<0.01);The treatment before and after 4W in group,12 W group,control group VAS pain score was7.31±1.23,5.16 ±1.34,3.98± 0.85;WOMAC score was 55.86 ±11.36,34.46 ±7.58,25.57 ±7.63;Harris score was 55.13 ±3.17,63.06±2.04,70.32 ±3;the experimental group of patients with VAS pain score was 6.66 ± 1.43,3.99±1.02,1.97 ±0.55;WOMAC score was54 + 3.88,21.78 ± 10.37.13.24± 7.12;Harris score was 54.29±1.91,66.66 ±3.31,86.17±4.26;the VAS scores and the WOMAC scores of two groups were gradually decreased,the Harris scores were gradually increased.Within the group compared the long-term curative effect was better than the short-term effect,the differences were statistically significant(P<0.01);and compared with the time between the two groups in the experimental group than the control group,the differences were statistically significant(P<0.01);That intra-articular injection of sodium hyaluronate therapy for hip OA patients can relieve pain symptoms,increase the activity of the hip and recover the function of hip joint,curative effect was better than the short-term curative effect,and the intra-articular injection of sodium hyaluronate combined with internal heat acupuncture treatment is better;ConclusionUnder the guidance of the image of the hip joint cavity puncture,it can be accurate positioning,improve the success rate of puncture.Intraarticular injection of sodium hyaluronate combined withhot needle therapy in the treatment of hip osteoarthritis can lubricating joints,vegetative cartilage,improving the activity of joints,restoring the normal balance of metabolism in the joints,and relieving the excessive tension of the soft tissue of joints and effectively alleviating the pain.In this study,articular injection of sodium hyaluronate combined with acupuncture group treatment of hip OA clinical curative effect,high cure rate,and significantly better than single intra-articular injection of sodium hyaluronate group,is a safe and effective treatment of high feasibility ideas,it is worthy of clinical application.
