Graves'disease (GD) is known as a organ--specific and systemic autoimmune diseases with about 0.5% occurrence of the total population that the female are at high risk.Current clinical performance is not limited to the thyroid gland, but rather a multi-system syndrome.It is made up of improved metabolic syndrome,diffuse goiter,Graves'ophthalmopathy?pretibial myxedema and Hyperthyroid heart disease et al.It is called diffuse goiter with hyperthyroidism since the majority of patients also have metabolic disorder and diffuse goiter,and is also known as Graves'disease.Now the basic method of controlling toxic diffuse goiter including:anti-thyroid drugs, radioactive iodine radionuclide therapy, surgery and embolization et al. Anti-thyroid drug and surgery treatment,the most widely used, while anti-thyroid drug therapy has disadvantange of a long treatment period,unclear indications of withdrawal, serious drug side effects, high relapse risk that the risk of surgery may result in permanent hypothyroidism.Therefore, to find a safe, effective, side few side effects of synthesized treatment method is still the key to GD.At present,therapeutics and researchers found clear heart fire and free flow of qi,a fundamental treatment, as initial therapeutics which stands out is that the prevention and reduction of adverse reactions, reducing the recurrence of hyperthyroidism.Study efficacy on the treatment of toxic diffuse goiter and the prevention of the early complications in GD patients by smoothing liver and clearing fire aims to provide a new method for hyperthyroidism to serve as the reliable evidence of clinical therapy.DanZhi compound is the representative formula.Previous clinical trials have been certified Danzhi compound cooperates with western medicine do have clear effect on hyperthyroidism.Both significantly controls metabolic disorder and reduce symptoms of diffuse goiter,and also has a definite effect on exophthalmos, meanwhile repairs the damage of ALT or AST in patients with hyperthyroidism.Surprisingly,it can decrease the incidence of drug side effects like peripheral blood, s neutropenia and the abnormal liver function.The experiment is divided into two parts:on one hand, we choose Ad-TSHR289/6xHis to immunize female mice BALb/c in order to replicate similar lesions of human autoimmune thyroid disease.In order to study DanZhi compound,s effect on the general state of the mice% the pathological damage of thyroid tissue and the positive influence of thyroid function via observing the weight of mice, body temperature, food intake,water intake and HE staining of thyroid tissue,s cells and measuring other biochemical changes ofTSH, FT3, FT4,TRAb.On the other hand,clinical observation of DanZhi compound aims to alleviate symptoms and recover the thyroid function on the hyperthyroidism patients with pathogentic fire derived from stagnation of liver-Qi.OBJECTIVE:Danzhi compound intervents experimental hyperthyroidism mice to study the effects of Danzhi Compound on thyroid function and morphology in experimental hyperthyroidism mice model. Further on the basis of the above result to make aclinical observation of the hyperthyroidism patients with pathogentic fire derived from stagnation of liver-Qi to explore these patients, symptoms and changes in thyroid function so as to assess the ratio of T3 and T4 whether to be one of the indicators of thyroid function recovery to reflect drug efficacy before or after the treatment.Clinical observations explore the feasibility of animal studies provide experimental evidence for further basic research.METHODS:Adenovirus Ad-TSHR289/6xHis immunes 6-week-old female BALb/c mice to replicate GD animal model, the cream of Danzhi compound and methimazole suspension to intervents immunized animals respectively after 10 weeks for 14 consecutive days, then remove the orbit blood, TSH, FT3, FT4 and TRAb and other laboratory parameters of blank group, model group and intervention groups were measured by ELISA.,comparing each experimental group of mice thyroid tissue HE staining, moreover recording pathological changes.70 patients hyperthyroidism patients with pathogentic fire derived from stagnation of liver-Qi admitted in Xie Chunguang, s outpatient were randomly divided into Danzhi compound group and the control group.Two groups were given basic treatment of hyperthyroidism, Danzhi compound group with another Danzhi compound free decoction, tid,100ml warm water for 4 weeks. Symptoms total score of the two groups were collected before and after 4 weeks,treatment and detect the efficacy of Danzhi compound for systematic evaluation by TSH, FT3, FT4, T3 and T4.RESULTS:1?GD mice models were treated with Danzhi compound1)Successfully constructed and purfied Ad-TSHR289/6xHis adenovirus:its titer is 1×1010granule/50ul/time;2)Successfully constructed GD hyperthyroidism model.After molding TRAb higher than the blank group was 42.3%(P<0.01)?Model groups FT4 levels and FT3 levels were higher than the blank group, Respectively,were25.6%(P<0.05)?13.9%(P < 0.05). The model group's pathological morphology of thyroid tissue on the light micriscope:Thyroid follicular part was high columnar,follicular colloid showed a small amount (individual) absorption vacuoles.3)Two weeks of drug intervention,Two drug group's TRAb concentrations were lower than the model group,Western medicine group and Chinese medicine group decreased,respectively,27.3%(P<0.01),12.7%(P< 0.05).Compare with model group,the levels of drug groups-FT3 and FT4 of increased (P>0.05).No significant differences between the two drug groups compared with the levels of FT4 and FT3 of the model group.The TSH levels of Western medicine group and Chinese medicine group after 2-weeks intervention increased compared with the model group,respectively,8.4%,21.8%(P>0.05).3)The two medicine groups, pathological morphology of thyroid tissue on the light microscope were same as the blank group.2?The clinical study of hyperthroidism patients with pathogentic fire derived from stagnation of liver-Qi1)The symptoms of improvement:the Danzhi compound group help relieve the hyperthyroidism spmptoms significantly(P<0.05).Both two groups, effective rate all were over 70%,the Danzhi compound group's inefficiency was 11.1% while the comparison group was 17.6%.2)The function of thyroid:before the therapy,the Danzhi compound group and comparison group,s patients were no difference in statistics (P> 0.05),but after four weeks,both of two groups TSH increased whie FT3 decreased without Statistical difference(P> 0.05).The FT4 of Danzhi compound decreased more evident than the comparison group(P<0.05).Conclusion:1?Selection of adenovirus Ad-TSHR 289/6xHis can successfully build an animal model GD hyperthyroidism mice;2?Smoothing liver and clearing fire Soothing, and medicine methimazole can adjust levels of GD mice, TRAb and improved thyroid pathological tissues, but the difference between the two groups was not statistically significant,.The next study needs to expand the size of sample.3?Danzhi compound can effectively relieve symptoms of hyperthyroidism patients with pathogentic fire derived from stagnation of liver-Qi, improving the levels of T3 and significantly better than methimazole group.
