Clinical Observation On The Treatment Of Diabetic Foot With Traditional Chinese Medicine Combined With Vacuum Sealing Drainage

Objective: To observe the clinical effect of the long-term application of traditional Chinese medicine prescription and the clinical experience of Soaking the feet combine with vacuum sealing drainage(VSD)on the treatment of diabetic foot(DF).Methods: 60 diabetic foot patients were enrolled in the study,all of them were diabetic foot wet gangrene and the grade was Wagner 3;Syndrome differentiation of traditional Chinese medicine(TCM)was syndrome of damp heat toxin.The patients were randomly divided into treatment group and control group with 30 cases in each group.Two groups of patients were given conventional treatment,such as diabetes health education,diabetes diet,controlling blood glucose,correcting water electrolyte acid-base balance,anti-infection and other symptomatic support therapy.On the basis of conventional treatment,the treatment group was treated with TCM of oral prescription,soaking the feet and combine with VSD.The oral prescription was named TANG ZU QING TUO and taken for 8 weeks.External treatment of Diabetic Foot Lotion 2,the general treatment of 5 to 7 days after the withdrawal of TCM immersion.Debridement and VSD was treated after the whole body condition was stable.Patients in the control group were treated with routine disinfection and dressing,hydrogen peroxide and 0.9% sodium chloride injection were used to wash the wound,Compound iodine disinfection and dressing change.This stage was 5 to 7 days,debridement and VSD was treated after the whole body condition was stable.The total course of treatment was 8 weeks.The number of white blood cells(WBC),C-reactive protein(CRP),the percentage of neutrophils(NEU%),ankle branchial index(ABI),hemodynamics of dorsal foot and so on before and after treatment was recorded.By comparing the changes of the disease,the curative effect was compared.Results: The treatment group cured 20 cases,markedly effective in 6 cases,effective in 3 cases,ineffective in 1 cases,the total effective rate was 96.67%.The control group cured 11 cases,markedly effective in 9 cases,effective in 7cases,ineffective in 3 cases,the total effective rate was 90%.The cure rate of the treatment group was 66.67%,the control group was 36.67%,the difference between the two groups was statistically significant(P<0.05).The wound healing rate of the treatment group was higher than the control group,and the healing time and hospitalization time of the treatment group were lower than those of the control group(P<0.05).Compared with the control group,the treatment group was better than the control group in ABI,blood flow velocity and vessel diameter of the dorsal artery.In the inflammatory indicators,such as WBC,CRP,NEU%,et al,the treatment group after treatment than the control group improved significantly.The treatment group of patients with amputation in 1 cases,the control group of patients with amputation in 3 cases.Conclusion: The treatment of diabetic foot with TCM combined with VSD can effectively improve the symptoms of diabetic foot,and it's not easy to appear toxic side effects and adverse reactions.Combination of TCM and Western medicine is an effective method for the treatment of diabetic foot.
