Objective:To analyse the clinical efficacy of vacuum sealing drainage(VSD)in the treatment of diabetic foot ulcer.Providing a reference for the choice of clinical diabetic foot ulcer treatment.Methods:A retrospective analysis of 134 diabetic foot patients treated in the Burn and Plastic Surgery department of the Fifth Affiliated(Zhuhai)Hospital of Zunyi Medical University from January 2014 to December 2019,from which 104 Wagner2-3 diabetic foot patients who met the exclusion criteria,50 males and 54 females.The patients were divided into VSD group and dressing group according to application of VSD.There are 26female patients and 26 male patients in the VSD group,but there are 28 female patients and 24 male patients in the dressing group.VSD group:patients were treated with VSD after surgical debridement.And the negative pressure to VSD is maintained between80mmHg to 125mmHg.VSD was continuously applied for 7 days.After the wound had no obvious purulent secretions and granulation tissue grew vigorously,the dressing was changed until the wound healed.Dressing group:patients were given a traditional dressing treatment after surgical debridement.The dressing was changed every 1-2 days until the wound healed.Other treatments:Both groups of patients were given conventional basic treatments such as blood glucose control,comorbidity control,anti-infection and microcirculation improvement.According to the hospitalization records and follow-up records,Age,ulcer area,the scores of wound bed scores,basal wound preparation time,healing time,average number of dressing changes,hospitalization costs and treatment effects were calculated for the two groups of patients.This study used?~2 test,repeated measures analysis of variance,t test analysis,logistics regression analysis,linear regression,rank sum test for comparative analysis of indicator data,with P<0.05 was statistically significant.Results:1.There was no statistically significant difference in the scores of wound bed scores after surgical debridement between the two groups(P>0.05);The scores of the wound bed scores on the seventh and fourteenth days of the groups were statistically significant(P<0.05).On the seventh and fourteenth days of treatment,the scores of the wound bed scores in the VSD group were higher than those in the dressing group.2.The average basal wound preparation time in the VSD group was(18.06±3.08)days,and the average basal wound preparation time in the dressing group was(23.96±4.10)days.There was a significant difference in basal wound preparation time between the two groups(P<0.05);the average healing time in the VSD group was(34.16±6.28)days,and the dressing group the average healing time was(49.48±7.03)days.There was a significant difference in healing time between the two groups(P<0.05).The average dressing change was(20.79±3.43)times in the VSD group.And there was(27.60±3.69)times in the dressing group.The difference in the number of dressing changes between the two groups was statistically significant(P<0.05).The average hospitalization cost of the VSD group was(12448.90±1822.03)yuan,and the average hospitalization cost of the dressing group was(8878.50±1902.60)yuan.There was a significant difference between the two groups(P<0.05).3.Age and ulcer area have a significant positive effect on the time of foundation preparation(P<0.05).The older patients are or the larger the ulcer area,the longer it takes to prepare for wound foundation.Age and ulcer area have a significant positive effect on wound healing time(P<0.05).The older the age or the larger the ulcer area,the longer the wound will be cured.4.Among the 52 patients,there are 26 cured cases,11 markedly effective cases,9 effective cases,and 6 ineffective cases in the VSD group.Among the 52patients,there are 13 cured cases,12 markedly effective cases,14 effective cases and 13ineffective cases in the dressing group.The difference was statistically significant(P<0.05).Conclusion:VSD for diabetic foot ulcers is more efficient and comfortable than dressing change,but the cost of hospitalization is higher.The dressing treatment time for diabetic foot ulcers takes longer than the VSD treatment,and it is less comfortable and less effective,but the hospitalization costs are lower.
