ObjectiveThe main aim of this paper is to discuss the security and curative effect about Argon-helium cryoablation and traditional Chinese medicine treating liver neoplasm,by researching the short-term effects and complications in liver neoplasm,researching long-term effects in primary hepatic carcinoma.Researching the variation of symptoms after argon-helium cryoablation and the relationship between spleen-deficiency syndrome and liver function,summarize the syndrome of TCM variation rule about argon-helium cryoablation,and discuss the advantage of combination therapy in liver neoplasm.Methods1.The short-term curative effect analysis about argon-helium cryoablation and traditional Chinese medicine treating liver neoplasm:According to the different part of the tumor in the liver,this paper researches by classifying lumps the high-risk group and the low-risk group among 86 cases accorded the position.Using Pearson chi-square test?independent sample T test?Fisher exact probability method and Mann-Whitney U rank test analyze the balance of the baseline by the type of data,compare the complication differences of two groups by the m RECIST criteria.2.The complications analysis:Accumulating the times of cryoablation in 86 ca,calculate the numbers of complications and related laboratory indicators before and after the cryoablation in two groups,compare the difference of the complications rate with Pearson chi-square test?Fisher exact probability method or paired samples t-test by the data.3.The long-term curative effect analysis:Researching 51 cases of primary hepaticprimary hepatic carcinoma in all follow-up cases,grouping by the diffence of position.neoplasm staging and size,contrast the difference of PFS and OS and survival rate of 6 months?lyear and 2 years between groups.Drawing the survival curves of PFS?OS by Kaplan-Meier method,compare the survival curves by Log-rank test,assess the long-term curative effect.Taking the OS as the end events of the research,assessing survival data by COX hazard regression,this paper also aims to explore the prognosis factors impacting the primary liver cancer.4.The syndrome of TCM variation rule about argon-helium cryoablation,and the correlation between spleen-deficiency syndrome of cancer and the function of liver:recording patients tongue and pulse picture?sign and symptoms to drew integration tables,measurement data with pairing t test and ranked data with Mann-Whitney U rank test.Results1.Tumor control probability:Evaluating the effect after cryoablationl month later by CT,MRI or CEUS with the m RECIST criteria,the CR of the 133 tumors from 86 patients included the study are 32,PR 75,SD 24 and PD 2.The rate of CR is 24.06%,with ORR(CR+PR)80.45%.There is not significant difference between the high-risk group and the low-risk group.2.Rate of complications:Complications in this subject cover hurts?vomiting?fever?ascites?pin-related blooding?coagulation disorders?sinus formation by skin frostbiting?raising of HBV-DNA?the incomplete ileus?hepatapostema?secondary infection and cold shock by 166 times cryoablations in 86 patients.There are 90times hurts in different degree,54.21%;fever 14times,8.43%;raising of HBV-DNA 14times,8.43%;pin-related blooding 11 times 6.63%;vomiting 7times,4.22%;coagulation disorders 6times,3.610,%;ascites 3 times,1.810%;sinus formation by skin frostbiting 3times,1.81%;the incomplete ileus 1 times,0.6%;secondary infection 2times,1.2%;hepatapostema ltimes,0.6%ALL of the complications can be controlled,and there are no other complications.Statistical tests show no difference between two groups besides hurts.The related indicators:The indicators such as ALT?AST?ALP?AFP?AFU are increased,while ALB?PA?HGB declined,other index such as PT?APTT?PLT?WBC?CEA?HBV-DNA no obvious change.3.Progression-free survival 3.1 Progress-free survival The study cavers 51 liver cancer cases.6months of PFS are 33 patients,64.7%;1 year are22,43.14%;2years are 7,13.73%.Grouping in different position,the median PFS is 9.000±2.200months vs 12.000±4.102months(P>0.05),revealing no remarkable difference between the high-risk group and the low-risk group.Grouping in different BCLC stage,the median PFS between BCLC A stage vs B stage vs C stage are 14.0±2.996months vs 7.0±1.871months vs 7.0±2.041months(P>0.05),revealing no remarkable difference between groups.Grouping in different size of tumors,the median PFS between d>5cm vs d?5cm are 8.00±1.265months vs 9.00±2.40months,revealing no remarkable difference between groups.3.2 Overall survival 6months of OS are 49 patients,96.08%;1 year are 42,82.35%;2years are 33,64.71%.the median OS is 31±5.59 months vs 33±3.604months(P<0.05),revealing no remarkable difference between the high-risk group and the low-risk group.Grouping in different BCLC stage,the median OS between BCLC A stage vs BCLC B stage vs BCLC C stage are 52.00±11.225months vs 32.00±7.128months vs 33.00±5.371,revealing remarkable difference between two groups.Grouping in different size of tumors,the median OS between d>5cm vs d?5cm are 31.0±4.989months vs 38.0±11.386months,revealing no remarkable difference between two groups.4.The Chinese medicine property of cryoablation:Syndrome of blood stasis is the main syndrome 1 week after cryoablation,which relates to D-dimer.cryoablation can ease spleen-deficiency syndrome and damp-heat syndrome of cancer.Spleen-deficiency syndrome show positive correlation with liver function.ConclusionsBoth the safety and effect are good by argon-helium cryoablation and TCM treat liver neoplasm in different position;cryoablation belongs cold nature as aggravating qi-stagnation?blood stasis?phlegm-damp,easing spleen-deficiency and damp-heat.The combination therapy deserves further promotion.
