Clinical Study On The "Reversal" Of Compensated Liver Cirrhosis

ObjectiveThrough this research:firstly,to discuss a new concept about the reversal of liver cirrhosis(generalized and narrow sense);secondly,on the basis of this concept,observing the reverse cirrhosis,and answering the impact on traditional Chinese medicine intervention for reversing liver cirrhosis or value as much as possible,as we provide a clinical assessment of liver cirrhosis clinical objective evidence of evidence-based medicine thought.MethodsFirst,through the study of domestic and foreign literature study,we analyze the change points and dispute cases of reversal of ideas.Second,in January 1,2006 to December 31,2015 in Guangdong Province Traditional Chinese Medical Hospital,Zhuhai hospital and University City hospital or department of gastroenterology outpatient or inpatient diagnosis for 99 patients with inclusion and exclusion criteria screening hepatitis B cirrhosis by the medical records a retrospective cohort study and analysis,statistical analysis of curative effect in the treatment of compensated hepatitis B cirrhosis with traditional Chinese medicine and Western medicine to evaluate the data by using statistical software,according to the clinical data of liver cirrhosis in the "reversal".ResultsThe first,through literature research,we understand the different views of scholars at home and abroad for reversing liver cirrhosis problems,and provide the basis and reason analysis of different points of view.We believe that the problems to explore and understand the reversal of liver cirrhosis from two aspects of narrow sense and broad sense:the reversal of liver cirrhosis narrow concept refers to the pathological changes of liver cirrhosis(morphological or histological nodules Jia Xiaoye)disappeared,accompanied by the disappearance of clinical symptoms and signs and liver cirrhosis;reverse concept is not only the broad sense refers to the pathological changes of liver cirrhosis(gross morphological or histological pseudolobule nodules)or stable remission,or even disappear,which contains a special reverse cirrhosis,it should also include the concept of TCM syndrome in patients with liver cirrhosis(including clinical symptoms and signs)and objective indexes(including serum blood routine,liver function,Coagulation,virology,etc.;imaging of ultrasound,CT,MRI,etc.),and evaluation criteria(including APRI,etc.)stabilized or improved.The second,the results of clinical case studies:In this study,99 cases were included,and there was no significant difference in gender and age.On liver function aspects,the three groups before and after treatment of ALT,AST,TBIL,ALB and PT in the group differences and the differences between the groups after treatment were not statistically significant(P>0.05).Blood routine,the three groups before and after treatment of WBC,RBC,HB,and PLT differences within the group after the treatment group had no statistical significance(P>0.05).In terms of APRI value,there was no significant difference between the three groups before and after treatment(P>0.05);and there were significant differences between the groups after treatment,high exposure group and low exposed group in traditional Chinese Medicine were lower than that in non Chinese medicine exposed group in traditional Chinese Medicine(P<0.05).Ultrasound score,exposure group before and after treatment was statistically significant difference of high school of medicine(P<0.05),traditional Chinese medicine non exposed group and low exposed group had no significant difference(P>0.05);and there were significant differences between the groups after treatment,high exposure group was lower than those in non Chinese medicine exposed group and low exposed group in traditional Chinese Medicine(P<0.05).The TCM symptoms of traditional Chinese medicine,low exposure group,the exposure group before and after treatment was statistically significant difference of high school of medicine(P<0.05),no Chinese medicine exposure group had no significant difference(P>0.05);and there were significant differences between the groups after treatment,low exposure group and high school Chinese medicine TCM syndrome score group compared with non exposed Chinese low exposure group(P<0.05).The virus,the three groups before and after treatment in group HBsAg and the difference between two groups after treatment(P>0.05).There were no significant differences between the three groups before and after treatment HBeAg and treatment groups(P>0.05).And there were no significant differences statistically significant differences between groups after treatment HBV-DNA,senior medical exposure group HBV-DNA negative rate than non exposed group and low in traditional Chinese medicine Chinese high exposure group(P<0.05).Conclusion1.The concept of reverse cirrhosis refers to the disease status of liver cirrhosis turn or development for the better,better perspective is divided into two aspects according to the pathology and clinic,both at home and abroad for the different views of cirrhosis can be reversed,in order to reduce the dispute,and in order to better assess the clinical therapeutic effect and prognosis of liver cirrhosis,unified understanding,we believe that the concept of reverse cirrhosis should be introduced and generalized.Narrow reversal reversal.2.Based on the analysis of clinical cases,from the perspective of the generalized concept of reverse cirrhosis,long term intervention therapy combined with traditional Chinese medicine and Western Medicine(especially more Chinese medicine intervention)of hepatitis B cirrhosis,replication inhibition of HBV DNA decreased sustainable,ultrasound score,improve liver fibrosis and TCM syndrome,and to a certain extent the "reversal" of liver cirrhosis;but for the narrow cases of cirrhosis reversal,it takes time and patience to observe and summarize.
