| Objective:The tutor found in clinical research in the treatment of gouty arthritis research,many patients with gouty arthritis in addition to inflammatory changes,urate also have varying degrees of damage to the kidney,through the observation of Chinese medicine compound BiNing Decoction in treating patients with early gouty nephropathy,prevention and treatment research of the prescription on renal the early damage of gouty arthritis and its possible mechanism.Method:By collecting the outpatient department of rheumatism of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from September 2015 to March 2017,the subjects were diagnosed as gouty nephropathy,a total of 30 cases.Randomly divided into treatment group(compound BingNing Decoction + western Allopurinol Tablets)and control group(Western medicine group,Allopurinol Tablets)respectively in 15 cases,one month for 1 courses,a total of 2 courses;two groups of patients in general,urine protein,urine occult blood,urine white blood cells,urine red blood cells;C-reactive protein,erythrocyte sedimentation rate;related indexes of blood uric acid and serum β2-microglobulin and urinary β2-microglobulin results by statistical processing and analysis etc..Results:1.The total effective rate of the treatment group was 93.3%,and the total effective rate of the control group was 66.67%.The treatment group improved obviously(P<0.05).2.TCM syndrome integral:before treatment,the treatment group compared with the control group,P>0.05,comparable.After treatment,the treatment group and the control group at two time points in the group were compared,P<0.01,significant difference.After treatment,the curative effect of the two time points between the two groups were compared,P<0.05,there were significant differences,the improvement of TCM syndrome score in the treatment group was better than that in the control group.3.Observation index:(1)Before treatment,blood uric acid group,P>0.05,no difference between the treatment groups;blood uric acid before and after treatment one mouth between and within the group compared to P<0.05,there was statistically significant difference;treatment of blood uric acid groups before and after treatment two mouths between groups were compared with P<0.01.There is a significant difference,statistically significant.(2)Blood P2-MG and urine β2-MG:blood and urine β2-MG among the 4 groups before the treatment with β>0.05,there was no difference in the results;after treatment,blood β2-MG and urine β2-MG treatment group compared to P<0.01,there was significant difference in blood and urineβ2-MG,P<0.05,compared with the control group,there was significant difference in blood P2-MG and urine P2-MG between the two groups were compared,P<0.05,effect of difference,that is better than the therapeutic effect in the treatment group and control group.(3)Urine protein,urine occult blood before and after treatment between the two groups compared with control group,P<0.05,the difference in the treatment group,there was statistical significance;the urine protein,urine occult blood in the treatment group and the control group were compared,P<0.05,shows that after treatment than before treatment to alleviate the symptoms,the treatmen was statistically significance;the red blood cells and white blood cells before the two groups were compared,P>0.05,no difference between the treatment group and the control;red blood cells and white blood cells respectively,P<0.05,differences have statistical significance;the treatment of red blood cells,white blood cells were in the corresponding group before treatment comparison,P<0.01,there is a significant difference.(4)C-reactive protein,erythrocyte sedimentation rate between each group compared with before treatment,P>0.05,with statistical significance;each group before and after treatment with P<0.05,there are differences between groups;each group after treatment with P>0.05,no statistical significance.Conclusion:Recurrent gouty arthritis can cause renal injury;traditional Chinese medicine compound BiNing decoction can cause clinical symptoms and signs of patients with gout can be effectively improved;the patient’s BUA level decreased,leukopenia,proteinuria and urinary occult blood in urine,decrease of Scr,blood β2-MG,urine β2-MG,24h urinary protein quantitation.The therapeutic effect was not obvious in reducing ESR and CRP.The compound Chinese medicine decoction for the treatment of gout has an exact clinical effect on early renal damage in patients with gout.It can be further studied and can be widely used in clinic. |
